Value Of Endoscopic Ultrasonography In Differential Diagnosis Of Gastric Stromal Tumors And Leiomyomas

Background and aims:With the popularization of endoscopic examination,more and more gastric stromal tumors and leiomyomas have been detected at an early stage and then been treated properly.With the analyzation of the different ultrasound endoscopic characters between gastric stromal tumors and leiomyomas,some endoscopic ultrasonography(EUS)features are detected with the comparison of gastric gastrointestinal stromal tumors and leiomyomas.Methods:Some EUS features in 92 patients who treated in Zhongda Hospital from January 2015 to December 2018 are evaluated.These patients are with gastric tumors confirmed by histopathologic diagnosis after endoscopic surgery.The EUS features are collected and analyzed by statistically and the sensitivity of specific features are tested.Result:Immuno-histochemical analyses demonstrated 63 gastric stromal tumors and 29 leiomyomas.There is no difference between gastric stromal tumors and leiomyomas in ulcer,lobulation,blood flow and border(P>0.05).Diameter larger than 2cm,homogeneity and rising from the forth layer have statistical significance(P<0.05).In the prediction of gastric stromal tumors,all enrolled tumors larger than 2cm are gastric gastrointestinal stromal tumors,the specificity of hetergeneity is high(specificity=86.2%)and the criteria of rising from the forth layer has a high sensitivity(SE=79.4%)Conclusion:There are differences between gastric stromal tumors and gastric leiomyomas in ultrasonography.Therefore,EUS can provide more evidence in the submucosal tumor differential diagnosis but is not an optional diagnosis method for submucosal tumors.
