Objective This study was carried out to investigate the prevalence of thyroid nodule(TN)and to explore the independent risk factors in elderly(age ?45 years)patients with type 2 diabetes mellitus(T2DM).Methods A total of 273 individuals completed this study,included 173 patients who are in the Department of endocrinology of our hospital with T2 DM as the T2 DM group,All patients with T2 DM met the criteria for diagnosis and classification proposed by the WHO diabetes expert committee(1999),100 elderly individuals with normal glucose metabolism who are At the same time go to our hospital physical examination center undergo oral glucose tolerance test and have Already been excluded diabetes as a control group(non-T2DM).We investigated thyroid ultrasonography properties of each subject and investigated the data of Clinical and laboratory :gender?age?height?weight?SBP?DBP?FPG?2-h glucose in OGTT?TG?HDL-C?Hb A1c?FINS?FT4?FT3?TSH.Results(1)The comparison of the general factors between T2 DM group and non-T2 DM group,FPG,2h PG,Hb A1 c,T2DM group was higher than that of non-T2 DM group(P<0.05),there was no significant difference between the other indicators.(2)Prevalence of thyroid nodules between T2 DM group and non-T2 DM group:T2DM group have 173 individuals,the number of people with thyroid nodules in this group is 92,the Prevalence of nodules was 53%,non-T2 DM group have 100 individuals,the number of people with thyroid nodules in this group is 32,the Prevalence of nodules was 32%,Compare The Prevalence of nodules between the two groups(53% VS 32%,X2=11.466 p=0.001),the difference was statistically significant.T2 DM Group and non-T2 DM group were divided into subgroups according to age and sex,Compare The prevalence of nodules in each subgroup:The prevalence in the two groups of male patients aged 45-59 years were respectively(29% vs24%,X2=0.183 P=0.669);The prevalence in the two groups of female patients aged 45-59 years were respectively(56% vs 33%,X2=3.081 P=0.079).The prevalence in the two groups of male patients over 60 years old were respectively(54% vs 26%,X2=5.663 P=0.017),the difference was statistically significant;The prevalence in the two groups of female patients over 60 years old were respectively(86% vs 50%,X2=8.496 P=0.004),the difference was statistically significant.(3)Patients with T2 DM were divided into two groups of TN group and non-TN group,The comparison of the general factors in two groups:female?age?SBP,TN group was higher than that of non-TN group(P<0.05),the difference was statistically significant.(4)The relationship of the prevalence of TN with gender and age in patients with T2 DM : 102 male patients with T2 DM,The prevalence in the two subgroups of male patients Aged 45-59 years and over 60 years old were respectively(29% vs 54% P=0.008),the difference was statistically significant;71 female patients with T2 DM,The prevalence in the two subgroups of female patients Aged 45-59 years and over 60 years old were respectively(56% vs 86%P=0.004),the difference was statistically significant;90 patients aged 45-59 years,The prevalence in the two subgroups of male and female were respectively(29% vs 56% P=0.01),the difference was statistically significant;83 patients over 60 years old,The prevalence in the two subgroups of male and female were respectively(54% vs 86 P=0.002),the difference was statistically significant.(5)Analysis the risk factors of thyroid nodule in patients with T2 DM :The Single logistic regression analysis shows Age(OR: 3.445,95%CI:1.838-6.457,P<0.05)?gender(OR: 3.794,95%CI:1.978-7.276,P<0.05)?HOMA-IR(OR: 2.884,95%CI:1.895-6.647,P<0.05)?BMI(OR: 2.253,95%CI:1.219-4.167,P<0.05)and SBP(OR:2.308,95%CI:1.241-4.292,P<0.05)are the risk factors of thyroid nodules.The multivariate logistic regression analysis shows Age(OR: 3.429,95% CI:1.669-7.046,P<0.01)?gender(OR:3.345,95% CI:1.641-6.816,P<0.01)and HOMA-IR(OR:2.485,95% CI:1.240-4.981,P<0.05)are the independent risk factors of thyroid nodules.Conclusion(1)The prevalence of thyroid nodule in elderly patients with T2 DM is higher than general population.Especially people are Female and older T2 DM patients.(2)Age,gender and HOMA-IR are the independent risk factors of thyroid nodules.
