Purpose:In this study,the clinical indicators of Type 2 diabetes mellitus(T2DM)patients with Thyroid nodule(TN)and T2DM patients without TN were compared and classified according to TCM symptoms and signs.To explore the related risk factors of T2DM with TN and distribution of TCM syndromes,so as to provide objective basis for the early prevention and treatment of thyroid nodules in diabetic patients.Material and method:In this study,T2DM patients hospitalized in the Department of Endocrinology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from December 2020 to November 2021 were selected according to inclusion and exclusion criteria,and were divided into T2DM group with TN and T2DM group without TN according to thyroid nodules.Clinical information of the 2 groups was collected,including gender,age,course of disease,Body mass index(BMI),Fasting,postprandial 1h and 2h venous blood was collected,and Fasting plasma glucose(Fasting plasma glucose)was determined.FPG,postprandial plasma glucose(2h PG),Fasting C-peptide(FCP),postprandial C-peptide(1H postprandial C-peptide,1h CP,2h CP(2 Hours postprandial C-peptide,2h CP),Free triiodothyronin(FT3),Free thyroxine(Free thyroxine,The Thyroid stimulating hormone(TSH),Thyroglobulin antibody(Thyroglobulin antibody)and Thyroid stimulating hormone(FT4)TGAb,Thyroid peroxidase antibody(TPOAb),Glycated hemoglobin(Hb A1c),Thyroid ultrasound examination report,TCM symptoms and signs were collected.Group of traditional Chinese medicine institute of Chinese medicine in 2011 for the prevention and control of diabetes in traditional Chinese medicine guide[1]combination of liaoning university of traditional Chinese medicine hospital endocrinology T2DM specification of clinical diagnosis and treatment,according to the patient’s symptoms and signs of traditional Chinese medicine is divided into hot and humid embodiment,qi and Yin deficiency syndrome,Yin deficiency syndrome,hot and humid embodiment and blood stasis,qi and Yin deficiency and blood stasis five TCM syndrome types.Observation Excel database was developed and statistical software SPSS 25.0 was used for data analysis.Measurement data were expressed as(?)S.Independent sample T test was used for nrmal distribution between two groups,one-way an OVA was used for normal distribution and homogeneity of variance between multiple groups,and non-parametric test was used for nonconformity.The statistical data were expressed by composition ratio N(%)and(?)2test.Logistic regression was used to analyze the risk factors of T2DM with TN,and analyze the distribution of TCM syndrome type of T2DM with TN and the correlation between TCM syndrome type and factors.P<0.05 indicated statistically significant difference.Results:1.Comparison of general conditions between the two groups:a total of 209 T2DM patients were included in this study,including 108(51.67%)T2DM patients with TN,and 101 (48.33%)T2DM patients without TN.Age,course of diabetes and BMI of T2DM with TN group were higher than T2DM without TN group,the differences were statistically significant(P<0.05).Women in T2DM group with TN were more than those in T2DM group without TN,and the difference was statistically significant(P<0.05).2.Risk factors analysis of T2DM group with TN:Logistic regression analysis showed that age and BMI were independent risk factors of T2DM group with TN(P<0.05).3.Distribution characteristics of TCM syndromes in T2DM group with TN:56 cases(51.85%)of qi and Yin deficiency combined with blood stasis syndrome>34 cases(31.48%)of dampness and heat trapping spleen combined with blood stasis syndrome>9 cases(8.33%)of dampness and heat trapping spleen syndrome>7 cases(6.48%)of Qi and Yin deficiency syndrome>2 cases(1.85%)of Yin and Yang deficiency syndrome.Conclusion:1.In type 2 diabetes patients,older age,longer course of disease and female patients are more likely to be associated with thyroid nodules.2.Increased BMI and age are independent risk factors for thyroid nodules in patients with type 2 diabetes.3.The syndrome of deficiency of qi and Yin combined with blood stasis is the most common TCM syndrome of type 2 diabetes accompanied by thyroid nodules,followed by damp-heat trapping spleen combined with blood stasis syndrome,and damp-heat trapping spleen syndrome.
