The Clinical Research Of The Early Diagnosis Of Urosepsis And The Early Diagnosis And Treatment In Women With Urosepsis During Pregnancy

Objective1?To analyze the early diagnostic value of PCT,IL-6,and CRP for urosepsis by examining serum level.2 ? To explore a surveillance system for early diagnosis and treatment in women with urosepsis during pregnancy.Methods1?From 2014 to 2016,105 patients with urinary tract infection in our hospital were divided into three groups: acute cystitis(14 males and 16 females,38-54 yrs),urinary sepsis(25 males and30 females,49-68 yrs)and urinary sepsis with shock(11 males and 9 females,55-74yrs).Neutrophil percentage in blood,white blood cell count in urine,serum levels of PCT,IL-6,and CRP were analyzed.ROC curve was applied to evaluate the diagnostic value for urosepsis.2 ? From 2010 to 2016,differential levels of these candidate markers were further retrospectively analyzed in pregnant women diagnosed as urosepsis(32 cases,23~31 yrs)compared to women with acute cystitis(32 cases,22~32 yrs).The diagnostic and prognostic significance for urosepsis were further summarized by examining levels of neutrophil percentage in blood,white blood cell count in urine,serum levels of PCT,IL-6,and CRP aimed to establish a surveillance procedure in pregnant patient with urosepsis.Results1?Serum levels of PCT,IL-6 and CRP were statistical different(P< 0.05)by comparing patients diagnosed as urosepsis to controls.PCT is the most valuable diagnostic marker among them,and its combined use with IL-6 or CRP would have a better AUC.2 ?There were statistical differences(P < 0.05)in levels of serum PCT,IL-6 and CRP between urosepsis group and cystitis group.The sensitivity of PCT,IL-6 and CRP for urosepsis was higher than that of neutrophil percentage in blood and WBC counts in urine.The levels of PCT,IL-6 and CRP were decreased in the 3rd days and 7th days after implantation of double-J stent(p <0.05).Conclusion1?PCT,IL-6 and CRP are useful for the diagnosis of urinary sepsis and are superior to the percentage of blood NEU and urine WBC count,PCT has a high single-value diagnostic value.The joint detection for levels of serum PCT,IL-6 and CRP is useful in diagnosis of urinary tract infection,evaluation the severity of the infection classification,2?The multi-index joint detection of PCT,IL-6 and CRP is useful for diagnosis,evaluation ofthe severity of the infection classification and treatment of urosepsis in pregnancy.Timely surgical treatment of the cavity and implantation of double-J stentcan could relieve obstruction of the middle and late pregnancy with urosepsis,it is conducive to infection control.
