Objective: In this study,we observed the general clinical features of patients with different types of obesity metabolism and compared the changes and significance of s CD163,TNF-alpha and CRP in different types of obesity metabolism,to investigate the role of s CD163 in the pathogenesis of different obesity metabolism and its influencing factors,to provide theoretical basis for further study on the pathogenesis of different types of obesity metabolism.Methods: Collected information include the general situation,history,family history,physical examination,smoking history,drinking history and other information from December 2015 to December 2016 in the first affiliated hospital of Xinjiang Shihezi University in various clinical departments of adult general.The concentration of blood collected in the morning fasting and using Roche automatic biochemical analyzer to detect the serum levels of fasting blood glucose,cholesterol,triglyceride,high density lipoprotein,low density lipoprotein and uric acid..According to the 2003 China guide of adults are overweight and obesity prevention and control of the definition of obesity diagnosis standard and NCEP ATP-Ⅲ defined metabolic abnormalities of the standard,the research object is divided into four groups: group of metabolically abnormal obese(MAO),group of metabolically healthy obese(MHO)and group of metabolically obese but normal weight(MONW),group of metabolically healthy and normal weight(MHNW).The concentrations of s CD163,TNF-α and CRP were determined by enzyme linked immunosorbent assay.Results: 1.The average age of all subjects was 63.6 ± 11.2 years old,and the mean age difference between the four groups was not statistically significant(p >0.05).2.The WHR of MHNW group was significantly lower than that of MHO group,MONW group,MAO group and MAO group(p <0.001).The SBP of MONW group and MAO group was higher than that of MHN group(p <0.001).The DBP of MAO group was significantly higher than that of MHNW group,MHO group and MONW group(p <0.001).The expression of Hb A1 C in MONW group and MAO group was statistically significant(p < 0.001),Hb A1 C,FBG,PBG,HDL-C and TG of MONW group and MAO group were higher than MHNW group and MHO group(p <0.001).The ALT of MAO group was higher than that of MHNW group and MONW group(p <0.05),the difference was statistically significant <0.05).The AST of MAO group was higher than that of MHNW group,MHO group and MONW group(p <0.05).The differences of TG,LDL-C,BUN and Scr between the four groups were not statistically significant(p> 0.05).3.The levels of s CD163,TNF-α and CRP in MHO group,MONW group and MAO group were significantly higher than those in MHNW group(p <0.001).The levels of s CD163,TNF-α and CRP in MAO group were higher than those in MHNW group,MHO group,MAO group,the difference was statistically significant(p <0.001).There was no significant difference in s CD163 and TNF-α between MHO group and MONW group(p = 0.307,p = 0.337).The level of CRP in MHO group was higher than that in MONW group(p <0.05).The effect of obesity on s CD163 was statistically significant(F = 358.546,p <0.001).4.s CD163 was positively correlated with TNF-α,CRP,BMI,WHR,SBP,DBP,Hb A1 C,FBG,PBG,TG and ALT,and negatively correlated with HDL-C(p <0.001).5.By using multiple stepwise linear regression method,s CD163 as dependent variable,the clinical indicators as independent variables,the higher BMI,Hb A1 C,DBP,and the lower HDL-C,the concentration of s CD163 was higher.Conclusion:There were inflammatory state among MHO group,MONW group,MAO group,and most obviously in MAO group.s CD163 may be involved in pathogenesis of obesity and metabolic.
