Objective To analyze the key factors affecting surgical outcomes of the retroperitoneoscopic adrenalectomy(RA)for giant adrenal tumorsMaterial and Methods From January 2007 to March 2015,35 retroperitoneoscopic adrenalectomy for giant adrenal tumors(?5cm)has been performed by one experienced surgeon. The influencing factors including age, tumor type, tumor maximum diameter and surgical outcomes including operative time, estimated blood loss, postoperative days, and conversion were retrospectively analyzed using. Results Totally,35 patients underwent RA successfully with 5 cases conversed to open.Age,pheochromocytoma and size are the three factors associated to conversion;Size and pheochromocytoma are the two factors in relation to operative time;Charlson comorbidity index(CCI)and pheochromocytoma are significant factors statistically to postoperative days; And age and pheochromocytoma are in relation to estimate blood loss.In logistic regression, the data showed that Log postoperative days=0.824CCI+1.891PHE-3.014(percentage correct 74.3%).Conclusions Retroperitoneoscopic adrenalectomy(RA)is applicable to giant adrenal neoplams more than 5 cm.And maximum diameter(MD)is in association with operative time and conversion.Phaeochromocytoma is related with conversion, operative time,EBL and postoperative days. And age is relevant to conversion and EBL.CCI is in association with postoperative days.
