Background:The first successful operation was performed by Roux and Charles Mayo in 1926(OA),OA has become a radical treatment of a lot of adrenal diseases. However, since Gagner performed the first report of laparoscopic adrenalectomy (LA) in 1992, LA has many advantages than OA has been found, the technology has been continually promoted and applied. With the improvement of living standards, people care more and more high quality of medical services, at the same time , the minimally invasive surgery are more and more popular and become the main trends of surgery. LA is considered the "gold standard " treatment of benign adrenal tumors .Objective: To investigate the safety and efficacy of Retroperitoneal Laparoscopic adrenalectomy (RLA) to treatment the adrenal lesions, and compare LA group with the open surgery group for analysis. Methods:To compare the clinical data of 91 adrenal lesion cases treated by LA (2 bilateral lesions) with 23 adrenal lesion cases (2 bilateral lesions) treated by OA for Retrospective analysis. The gender, age, drainage tube indwelling time, mean blood loss, postoperative time into the liquid, tumor size, postoperative bed time, postoperative body temperature, postoperative hospital stay and complication rate of the two groups were Compared.Results:91 cases in LA group were operated successfully; 21 cases of 23 patients in OA group successfully completed, 2 cases can not be completed because of the tumor's dense adhesions with the surrounding tissues and organs. The result is no statistical difference between the two groups in gender, age, the time of drainage tube indwelling; but there is statistically significant differences between the two groups in the mean blood loss, the time of postoperative consuming liquid diet, the tumor size, the postoperative bed time, the postoperative body temperature, the postoperative hospital stay and the complication rate and so on. They show that laparoscopic surgery is superior to open surgery, except the data of tumor size. Conclusions: The results show it is safe, effective, small trauma, rapid recovery, little bleeding and complication that laparoscopic resectiong of benign adrenal tumor and simple adrenal metastatic cancer. For the skilled and experienced surgeon, It will become the trends that LA takes the place of OA in surgical treatment procedures for adrenal diseases.
