Objective:The aims of study are to investigate the epidemic status of obesity and hypertension among middle-aged and elderly population in china,For further evaluate the association between obesity and hypertension,and compare the predictive effect of BMI,WC and WtHR on hypertension,trying to find an index which can performance better on predictive the risk of hypertension.For primary health institutions carry out more effective prevention and control strategies on obesity and hypertension related diseases provide some basic research supports.Methods:Using 2013 CHARLS data,a total of 18605 participants among middle-aged and elderly in china were selected through a population proportionate sampling from 10624 households,removed the participants who do not take a part in physical examination,also removed the participants who's HuKou status was Unified Residence HuKou,finally,there were 11365 individuals selected for the analysis.Firstly,Chi-square test used to compare the prevalence rate of obesity and hypertension between different groups.Second,Pearson correlation coefficient used to describe the liner relationship between obesity indexes and blood pressures.Third,One-way analysis of variance used to compare the mean value of SBP,DBP between vary groups.Analysis the association between obesity indexes and the risk of hypertension by binary logistic regression,OR(95%CI)values was calculated;Standardized the obesity indexes(BMI,WC and WtHR)in order to further investigate the correlation between the risk of prevalence rate of hypertension with obesity indexes while changed 1sd.Logit regression and ROC curve were used to analysis the predictive effects of obesity indexes on hypertension.Results: 1.The prevalence of obesity among middle-aged and elderly population in 2013 was 13.2%(men 10.5% vs women 15.7%).The prevalence of obesity initially declining with increasing age,The prevalence of obesity was higher among population which was female,the marital status is married,well-educated,non-agricultural HuKou,non-smoke and drink habits,respectively.2.The prevalence of central obesity was 49.1%(men 37.4%,vs.women 59.7%),The prevalence of central obesity was also initially declining with increasing age.The prevalence of central obesity was higher among population which was aged 45~64 years,the marital status is married,the level of education is Middle-school and above,non-agricultural HuKou,non-smoke and drink habits.3.The prevalence of hypertension among middle-aged and elderly population in 2013 was 30.0%(men 30.6%,vs.women 29.4%),respectively.With the age increasing,the prevalence of hypertension been risen.The prevalence of hypertension was higher among population which was married,the HuKou status was non-agricultural,had a habits of smoke or drink,obesity and central obesity.There was a reverse relationship between the level of education and the prevalence rate of hypertension.And also the female minority individual's prevalence of hypertension was much higher.4.The Person correlation analysis results showed that,after adjusted for ages and gender,BMI,WC and WtHR were positively associated,respectively.Especially,WC was closely associated with WtHR(r=0.98);BMI,WC and WtHR also were positively associated with SBP,DBP(p(27)0.01),Especially,WtHR was closely associated with SBP(r=0.22).With the level of obesity indexes groups from low to high,the prevalence rate of hypertension was rising,compare with the WHtR<0.50 group of female,the OR of WHtR?0.50 for the risk of the prevalence rate of hypertension were 2.05(95% CI = 1.71 ~ 2.46).WtHR performed better than BMI and WC on predicted the prevalence risk of hypertension,the AUC value was 0.611.Conclusions: 1.The epidemic status of obesity among middle-aged and elderly in 2013 were already reached a severe level in China.High risk population for obesity were aged among 45~64 years,the marital status was married,the level of education was under Middle-school and non-agricultural HuKou 2.The prevalence of hypertension among middle-aged and elderly were reached pretty seriously level in China.High risk population for hypertension were aging,the marital status was non-married,minority nationality,the level of education was under Middle-school,non-agricultural HuKou,had habits of smoke or drink,obesity and central obesity.3.There was a closely association between obesity and hypertension,WHtR can be used as better predictors in screening high risk population of hypertension for the primary health institutes.Suggestion: 1.Primary health institutes should appeal all citizens to hold the ideals of maintaining a healthy weight and mobilize the public to adopt a healthy lifestyle in order to achieve effectively prevention and control of hypertension and its related diseases.Meanwhile,Primary health institutes also should actively intervene the overweight population among middle-aged and elderly for better control the scale of obesity population.2.Primary health institutes also should actively develop and implement the strategies of prevention and treatment of hypertension in middle-aged and elderly population,improve the control rate of hypertension,reduce the economic burden which caused by the disease.
