Objectives:We aimed to discuss the relationship between different types of obesity and the prevalence of hypertension by observing the influence of the number of abnormal obesity index (bod mass index, waist circumference, waist-hip ratio and waist to height ratio) on the risk of hypertension.Methods:In the health care center of Xiangya Hospital,961cases including gender, age, family history, past history, smoking history, blood pressure, height, weight, waist circumference, hip circumference are collected. Then they are divided into5groups:normal group,1index abnomal group,2indexes abnomal group,3indexes abnomal group and4indexes abnomal group according to the number of abnormal obesity index(bod mass index, waist circumference, waist-hip ratio and waist to height ratio). Compare differences in the prevalence of hypertension between each group using chi-square test and compare the risk of hypertension between each group using binary logistic regression correlation analysis after adjusting for age, gender, family history, smoking history, uric acid, fasting blood glucose, total cholesterol and triglyeride.Results:The prevalence of hypertension in the1index abnomal group,2indexes abnomal group,3indexes abnomal group and4indexes abnomal group are16.1%,21.1%,27.1%and48.5%respectively, which is significantly higher than9.0%of which in normal group, and the difference is statistically significant (P<0.001);(2) Binary logistic analysis shows that the1index abnomal group(OR=1.418,95%CI:1.078-2.984, P=0.030),2indexes abnomal group(OR=1.600,95%CI:1.142-3.581, P=0.025),3indexes abnomal group(OR=2.042,95%CI:1.382-3.818, P=0.011)and4indexes abnomal group(OR=3.557,95%CI:2.139-5.15,.P<0.001)are suffering from higher risk of hypertension than the normal group after adjusting for age, gender, family history, smoking history, uric acid, fasting blood glucose, total cholesterol and triglyeride. Conclusions:The risk of hypertension grows higher according the the number of abnormal obesity index (body mass index, waist circumference, waist-hip ratio and waist to height ratio). Objectives:We aimed to discuss the effect of obesity on the target organ damage of hypertension by observing the effect of obesity on left ventricular mass index (LVMI), estimated glomerular filtration rate (eGFR), brachial-ankle Pulse wave velocity (baPWV) and ankle-brachial index (ABI).Methods:In the health care center of Xiangya Hospital,661cases including results of renal function, echocardiography, and arterial elasticity are collected and LVMI, eGFR, mean baPWV, and mean ABI of each are calculated. Then divide them into3groups according to their history of hypertension and BMI:Control (Cons) group, hypertensive (HT) group, and hypertension and obesity (HT+OB) group. Cons group includes283cases, HT group includes92cases and HT+OB group includes286cases. Compare differences of LVMI, eGFR, baPWV and ABI among3groups using analysis of covariance. Analyze single factor that affect LVMI, eGFR, PWV and ABI using Pearson correlation, which is then used as independent variables and LVMI, eGFR and PWV as dependent variable respectively to study influencing factors of LVMI、 eGFR and PWV using stepwise linear regression analysis.Results:①The LVMI of the HT+OB group is82.21g/m2, and it is significant higher than that of70.28g/m2in Cons group and76.53g/m2in HT group, and the difference is statistically significant (P<0.05); The eGFR of the HT+OB group is80.03ml/mm/1.73m2, and it is significant lower than that of91.44ml/min/1.73m2in Cons group and85.53ml/min/1.73m2in HT group, and the difference is statistically significant (P<0.05); The baPWV of the HT group and HT+OB group are1645.5cm/s and1462.2cm/s respectively, which is significant higher than that of1243.5cm/s in Cons group, and the difference is statistically significant (P <0.05); whereas the baPWV of the HT+OB group is1462.2cm/s, and it is significant lower than that of1645.5cm/s in HT+OB group, the difference is statistically significant (P<0.05).②Stepwise linear regression analysis shows that factors affect LVMI includes age, gender, BMI and hypertension; factors affect eGFR includes age, gender, BMI, hypertension and uric Acid; factors affect baPWV includes age, gender, BMI and hypertension and BMI is a protective factor。No significant relevant is find between BMI and ABI by single factor correlation analysis(P>0.05).Conclusions:Obesity can aggravate left ventricular hypertrophy and renal insufficiency in hypertension patients. Whereas it may have a protective effect on the elasticity of large artery.
