One Case Report Of Drug-induced Liver Injury And Literature Review

Objective: To investigate the etiology,pathogenesis,related examination,diagnosis and prognosis of jaundice and drug-induced liver injury,in order to improve the diagnosis and treatment of this disease.Methods: To analyze the clinical data of one case of drug-induced liver injury,analyze and summary the literature.Data: an elderly female patients,fatigue,tired of oil,bloating,skin yellow dye for 10 days.Physical examination: sclera and systemic skin yellow dye.Interventions: maintain liver function,fade yellow and provide energy were given after diagnosis were confirmed following exclusive method and conventional examinations,hepatitis virus,respiratory syncytial virus,ceruloplasmin,ferritin,serum iron,tumor markers,coagulation,cytomegalo virus,thyroid function,autoantibody,abdominal color Doppler ultrasound,upper abdomen enhanced MRI + MRCP and other tests.Results: following the conventional examinations the common cause of jaundice were excluded.The upper abdomen enhanced MRI + MRCP examination,suggesting: many liver cysts,the liver diffuse abnormality signal,bile duct no expansion,increased gallbladder,the signal within the uniform.On the basis of exclusion liver cancer and bile duct disease,maintain liver function,fading yellow and providing energy were given to diagnostic treatment.Dynamic monitoring of liver function,alpha-fetoprotein.combined the result of liver enzymes,bilirubin gradually decreased normal,alpha-fetoprotein first rise and fall,with the history of medication before a month,the patient was diagnosis as drug-induced liver damage.Conclusion: drug-induced liver injury(DILI),clinical manifestations and test results are not specific,is a disease which drugs or their metabolites lead to abnormal liver function.The clear history of medication,associated with the time of liver function abnormal,is main feature.In general,the diagnosis of drug-induced liver damage depends on exclusion.Although most of the symptoms of mild liver disease can be self-healing,but if not promptly diagnosed,given symptomatic treatment measures,acute liver failure,high mortality,can be caused.Therefore,strengthen its understanding,early diagnosis,timely treatment,improven the prognosis is needed.
