Investigation of the value of upper-arm pain during throwing training in predicting throwing fracture Objective: By exploring the correlation between upper arm pain caused by throwing and throwing fracture during training period,we tried to analyze the factors,including pain degree,lasting time and relief method of the upper arm,which could help to predict throwing fracture.It will provide evidence for screening soldiers with high risks of throwing fracture during training period.Methods: A case-control study was conducted on the recruits admitted to No.150 Hospital of PLA from January 2015 to March 2016.A total of 30 patients diagnosed as humeral fractures by X-ray were included in the case group,and 60 recruits with throwing fracture determined by X-ray during the throwing training period were included in the control group.The factors including age,education level,birthplace,pain degree,lasting time of pain and relief method,which were able to affect the throwing fracture,were investigated by questionnaire.The statistical data were analyzed with chi-square test and two independent samples t test.Results: There were significant differences in the degree,lasting time and relief method and the number of patients with the swollen arm between case group and control group.The degree of pain in the throwing arm in case group was significantly greater than that incontrol group(p < 0.05).The lasting time of pain of throwing arm in case group was significantly longer than that in control group(p < 0.05).The number of patients who relieved the pain with drug was significantly more than that of the control group(p < 0.05).Additionally the number of patients with swollen arms in the case group was significantly more than that in the control group(p < 0.05).Conclusion: During the recruits' training period,the person who had a swollen arm and pain of the throwing arm that lasted more than 1 week and could be relieved by drug was in high risk of throwing fracture.The person needed to rest and the training program needed to be adjusted to avoid throwing fracture.Part ? LC-DCP Versus Minimally Invasive Lag Screw in Treatment of Throwing Fracture Objective: To evaluate the effectiveness of LC-DCP and minimally invasive lag screw for humeral shaft fracture.Methods: From December 2013 to February 2016,the clinical data of 37 patients with humeral shaft fractures were retrospectively analyzed.Among them 29 case were treated with LC-DCP,and 8 cases were treated with limited open reduction and tension screw fixation.There was no significant difference in gender,age,education level before joining army and time from injury to operation between the two groups(p > 0.05).The regular clinical examinations and X radiography were done.Elbow function was evaluated by MEPS after surgery.Results: The operation time in LC-DCP group was significant more than that in lag screw group(p < 0.05).The intraoperative blood loss in LC-DCP group was significant more than that in lag screw group(p < 0.05).There was no significant difference in hospitalization time between the two groups(p > 0.05).All patients were followed up 12 months on average(range,9-36 months).At 6 months after operation,the bone healing rates of the two groups were 100%,showing no significant difference.The bone healing time was 9.93±3.05 weeks in LC-DCP group and 11.00±2.33 weeks in lag screw group,showing no significant difference(p > 0.05).Between LC-DCP group and lag screw groups,no significant difference was found in radial nerve injury(2 cases in LC-DCP and 1 case in lag screw)and elbow function(p > 0.05).There were no complications such as internalfixation failure,infection and bone nonunion after operation.Conclusion: Both LC-DCP and lag screw applied for humeral shaft fractures achieved satisfactory results.More attention should be paid to avoid blood supply destruction and protect radial nerve in the fixation of LC-DCP.
