Objective:Through the investigation of the subjective sleep quality of hypertensive patients in the palm tree community in Kunming,to explore the influence of sex,age,career,education level,retirement,previous physical illness history,economic level and blood pressure control level on the prevalence of sleep disorders in hypertensive patients,and to provide reference for community management and prevention of hypertension.Methods:From January 2017 to April 2017,investigate in a total of 450 hypertensive patients who have established health records at the Palm Tree Camp Community Health Service Center.33 people were excluded from the information because of incomplete information.A total of 417 patients were enrolled in the community clinic.Survey including gender,age,career,education level,retirement,previous physical illness history,economic level,blood pressure control level,subjective sleep quality,anxiety and depression.Assessment of subjective sleep quality using the Pittsburgh Sleep Quality Index(PSQI)scale,the PSQI scale is currently used to assess subjects' subjective sleep quality in nearly one month,including 19 self-rated entries and 5 assessment by others,of which the last 19 self-rated entries and five assessment by others do not participate in the score,a total score of 0 to 21 points,the higher tihe score,said the worse the quality of sleep.0 to 6 divided into no sleep disorders,7 to 11 divided into mild sleep disorders,12 to 16 divided into possible sleep disorders,17 to 21 divided into definite sleep disorders.Anxiety and Depression assessment using the Hospital Anxiety and Depression Scale(HADS),HADS has a high degree of reliability and validity not only in the application of outpatient clinics in the general hospital,but also in the health checkup.It consists of 14 entries,of which 7 items were assessed as depression and 7 items were assessed as anxiety.A total of 6 reverse questioning entries,5 in the Depression Subscale,1 in the Anxiety Subscale.Depression/anxiety score of 0 to 21 points:0 to 7 divided into no depression/anxiety,8 to 10 divided into possible depression/anxiety,11 to 21 divided into definite depression/anxiety.According to statistical analysis,whether the prevalence of sleep disorders in hypertensive patients is related to gender,age,career,education,retirement,previous physical illness history,economic level and blood pressure control level.Results:1.This study showed that all sleep disorders were 189 cases in 417 hypertensive patients,the prevalence rate is 45.3%.2.There was a significant difference in the prevalence of sleep disorders among different sexes in hypertensive patients(P<0.05),The prevalence of sleep disorders(51.9%)in female hypertensive patients was higher than that in male hypertensive patients(36.5%),and the prevalence of mild,moderate and severe sleep disorders was 25.1%,22.6%,4.2%(24.2%,10.1%,2.2%),which the prevalence of moderate sleep disorders was the most significant.3.There was no significant difference in the prevalence of sleep disorders between hypertensive patients under 65 years of age(40.2%)and those with hypertension at age 65 and above(47.7%)(P>0.05).4.There was no significant difference in between hypertensive patients with mental and physical labor(including those who had retired,distinguish by their work before the retired)(P>0.05).The prevalence of sleep disorders of them is46.1%?44.35.5.There were statistically significant differences between retired and non-retired hypertensive patients(P<0.05).Retired hypertensive patients including those who had previously worked for example farmers,small traders,etc.who were no longer engaged in any occupation.The prevalence of sleep disorders(46.1%)was significantly higher than that of non-retired hypertensive patients(35.5%).6.There was a significant difference in the prevalence of sleep disorders in hypertensive patients with different educational levels(P<0.05).The prevalence of sleep disorders was 46.2%(primary and below),48.7%(secondary),37.6%(college and above).7.There was a significant difference in the prevalence of sleep disorders among hypertensive patients with physical illness history(P<0.001).The prevalence of sleep disorders(58.1%)in hypertensive patients with physical illness history was significantly higher than that in hypertensive patients without physical illness history(36.3%).8.There was no significant difference in the prevalence of the sleep disorders between the average monthly income in hypertensive patients of less than 3,500 yuan or less than it(48.0%)and those who had the average monthly income of more than 3,500 yuan(39.0%)(P>0.05).9.There was no significant difference in the prevalence of sleep disorders between hypertensive patients who with SBP<140mmHg and DBP<90mmHg and hypertensive patients who with SBP>140mmHg and/or DBP?90mmHg(P>0.05).Conclusion:1.The prevalence of sleep disorders in female hypertensive patients was higher than that in male hypertensive patients.The prevalence of moderate sleep disorder was significantly higher than that of male patients,and the fluctuation of blood pressure in female hypertensive patients who decreased pressure drug effect is poor,it should pay attention to its sleep.2.The prevalence of sleep disorders was significantly higher in retired hypertensive patients(including those who had no previous work for example farmers,small traders,but no longer engaged in any occupation).Blood pressure control in retired hypertensive patients should be combined with their sleep conditions for comprehensive analysis.3.There were differences in the prevalence of sleep disorders among hypertensive patients of different educational levels,primary school and below(46.2%),secondary school(48.7%),university and above(37.6%)(P<0.05).For the prevalence of sleep disorders,the prevalence of sleep disorders was the highest in primary school and below,Followed by secondary school,university and above least.The results suggest that the degree of education affects the prevalence of sleep disorders in hypertensive patients,secondary school education,especially junior high school education in patients with hypertension should pay more attention to the quality of sleep.4.There was a significant difference in the prevalence of sleep disorders among hypertensive patients who had previous physical illness history compared with those without.5.In this study,the prevalence of sleep disorders in hypertensive patients with age,occupation,blood pressure control level,economic income no correlation,need to expand the sample size to further study.
