Objectives To explore the detection rate and influencing factors of OSAS among elderly hypertensive snoring subjects.Methods This is a cross-sectional study. A total of 298 hypertensive patients attended Berlin questionnaire, 197 patients who were at high-risk for OSAS completed polysomnography monitoring for a whole night.OSAS was defined by clinical symptoms and apnea-hypopnea index(AHI)not less than 5 per hour. Detection rate of OSAS were analyzed and clinic data of OSAS and non-OSAS group were compared. Stepwise linear regression and Logistic regression analysis were used to analyze the relevant factors correlated to AHI and OSAS with hypertension.Results 1 Of the 197 hypertensive patients(122 were males)were 59.47(±10.47)years, 165 were OSAS. The detection rate of OSAS in whole hypertensive patients was 83.8%, 89.3% in males and 74.7% in females, respectively. That of light, medium and severe OSAS was 52.3%, 20.8% and 10.7%, 50.8%, 27% and 11.5% in males and 54.7%, 10.7% and 9.3% in females respectively. The detection rate of OSAS in male was higher than women(x2=7.355; P=0.007). That of medium OSAS in male was higher than women(x2=7.564; P=0.006). That of light and severe OSAS had no gender difference(P > 0.05).2 The detection rate was 56.9% in normal weight patients, 92.9% in overweight patients, and 100% in obesity patients respectively(x2= 44.637; P < 0.001); The detection rate was 75%in normal waist circumference group and 88.8% in centricity obesity group, respectively(x2= 6.395; P = 0.011); The detection rate in hypertension level 1, level 2, level 3 groups was 57.5%, 98.9% and 100% respectively(x2= 48.632; P < 0.001). 3 BMI[(26.37±2.77)kg/m2], SBP[(159.59±14.42)mm Hg], DBP[(98.71±9.4)mm Hg], LDL[(2.85±0.82)mmol/l] and Waist circumference [(90.56±8.62)cm] in OSAS groups were more higher than non-OSAS group[(22.43±2.04)kg/m2,(146.83±4.6)mm Hg,(85.51±7.51)mm Hg,(2.22±0.6)mmol/l,(81.84±8)cm].There were statistical differences between the two groups(t value were-7.654,-4.945,-7.49,-4.11and-5.289; P < 0.05). BMI,SBP and DBP were positively associated AHI(r =0.839; P < 0.05). BMI、SBP and DBP were increased risk of OSAS in hypertension[the OR values(95%CI) were 2.554(1.408-4.632), 1.432(1.154-1.777), 1.286(1.058-1.562)].Conclusions 1 The detection rate of OSAS was high among elderly hypertensive snoring subjects, and had gender and age difference. 2 With the increase of body mass index, waist circumference, blood pressure, grading, the detection rate and the severity of OSAS in hypertensive patients increased. 3 High BMI、SBP、DBP were associated with OSAS in hypertension.
