BackgroundRadiofrequency catheter ablation is the most effective treatment of arrhythmias, and has been widely applied in clinical practice. To prevent the vascular complications such as bleeding and hematoma, the patients are restricted to bed rest with absolute immobility of leg, which can lead to their discomfort and intolerance, such as lower back pain, numbness of limb, loss of appetite, urinary difficulty, and also make patients to be anxious and agitated after ablation. However, the risk of bleeding after ablation through the femoral vein is low, prolonged bed-rest time is not necessary. The patients can change position and get out of bed earlier, which is helpful to promote the early rehabilitation of the postoperative patients.AimThis study aimed to investigate the effect of early ambulation on patients after ablation, which help to provide a safe protocol that promote patients' comfort without increasing the risk of vascular complications.Through the nursing intervention measures of early ambulation to improve the level of comfort, improve patients' self-care ability and help them establish confidence, reduce the risk of complications, reduce length of the hospital stay and the cost of nursing care, and provide evidence-based basis for clinical practice.MethodsIn this study,120 patients at a comprehensive teaching hospital in Guangzhou were randomly assigned to either the control group or the two experimental groups(A and B).Patients in the control group received the usual care, remaining bed rest for 8 hours with the affected leg immobilized at least 4 hours.Patients in Group A ambulated at 4 hours with the affected leg immobilized, and patients in Group B changed position after ablation and ambulated after 4 hours bed rest. The severity of bleeding, hematoma, back pain, the level of anxiety and comfort were recorded at regular intervals after the ablation.ResultsThere were no significantly statistical differences between the groups in the incidence of bleeding and hematoma.Incidence of urinary discomforts, numbness of limb, loss of appetite and severity of back pain were higher in the control group. The patients in the experimental group had significantly higher comfort and lower anxiety levels than the control group.Compared with the control group, the overall comfort level of experimental group B is the best.ConclusionsThe study shows that changing the body position and early ambulation after ablation procedure can be safe, they had no remarkable effects on the incidence of vascular complications and may reduce discomforts. The application of change position in bed after ablation and ambulate after 4 hours is safe, which is effective in reducing the complications related to prolonged bed rest.
