Purpose: The level of IgG、IgM、IgA、CD4+、CD8+ after birth immediately and after 6 months ,delayed umbilical cord camping (45 -60s) and immediately(< 10s) were compared,to explore the effect of delayed umbilical cord clamping on newborn immune function, so that providing the theoretical basis to improve the immune function of the newborn.Methods: From September 2014 to December 2015 ,100 cases of pregnant women in the 107th Hospital of PLA were randomly devided into two group , delayed umbilical cord camping (45 -60s)and immediately(<10s). After delivery,the newborn were placed on the tray 10-20cm below the obstetric table. 10 ml umbilical venous blood sample was drawn in the umbilical vein that was exposed clearly and disinfected with iodophor by the assistant with sterile gloves after cutting the umbilical cord :4ml was injected into the negative pressure blood collection tube contains 3.6 mg two triethylamine four acetic acid potassium and shook up,IgG, IgM, IgA were measured with immune turbidity assay;2ml was injected into EDTA.K2 anticoagulant tube, CD4、CD8 were detected by flow cytometry .The level of them were detected by the same method during 6 months follow-up. Analyses were conducted on the data using statistical software SPSS 22.0,measurement data were expressed as mean±standard deviation,deviation of that group differences between single-sample t test of variance, rate was shown by percentage and was compared with by x2 test,P<0.05 indicated statistical difference.Results:(1) IgA: The level of IgA in neonates at birth was (0.12 ± 0.03)g/L in the delayed umbilical cord camping group and the immediately, respectively. There was no significant difference of the level of IgA between the two groups (P=0.78) . After six months, the level of IgA in neonates was(0.18±0.09) g/L and (0.17±0.06) g/L in two groups, respectively. This difference was not statis-tically significant(P=0.83). However, IgA was increased at six months of age compared with at birth.(2) IgM:The level of IgM in neonates at birth was( 0.14±0.05)g/L and (0.13±0.07)g/L in the dela yed umbilical cord camping group and the immediately, respectively. There was no significant diffe-rence of the level of IgM between the two groups (P=0.34) . After six months, the level of IgM in neonates was (0.50±0.21) g/L and (0.62±0.29) g/L in two groups, respectively. This difference was not statistically significant (P=0.27 ) . However, IgM was increased at six months of age comp-ared with at birth.(3) IgG:The level of IgG in neonates at birth was (10.24±2.06) g/L and (10.61±2.08) g/L in the delayed umbilical cord camping group and the immediately, respectively. There was no signific-ant difference of the level of IgG between the two groups (P=0.62) . After six months, the level of IgG in neonates was (4.25±0.34) g/L and ( 3.26±0.37) g/L in two groups, respectively. This diffe-rence was statistically significant (P=0.04) . However, IgG was reduced at six months of age comp-ared with at birth.(4) CD4+:The level of CD4+in neonates at birth was (46.10±6.95) % and (44.65±5.87) %in the delayed umbilical cord camping group and the immediately, respectively. There was no signif-i-cant difference of the level of CD4+ between the two groups (P=0.48) . After six months, the lev-el of CD4+ in neonates was (43.40±5.62) % and (40.20±3.91) % in two groups, respectively.This difference was statistically significant(P=0.04).(5 ) CD8+:The level of CD8+ in neonates at birth was (18.55±3.24)% and (18.20±3.87)% in the delayed umbilical cord camping group and the immediately, respectively. There was no significant difference of the level of CD8+ between the two groups(P=0.76). After six months, the level of CD8+ in neonates was(26.10±3.75)% and (22.85±4.67)% in two groups, respectively. This difference w-a s statistically significant(P=0.02).Conclusion: More immune factors and immune cofactors in umbilical cord blood was into neonatal body by delaying umbilical cord clamping,so that the degree of IgG and CD8+ was increased.Helping to improve the immune function of neonates ,reducing the mortality and fatality rate of newborns.
