Purpose: Acute optic neuritis is an acute inflammatory demyelinating disease of the optic nerves.Approximately 30% of optic neuritis patients have one or multiple episodes of recurrence within 5 years. As far as we know, published analyses lacked race study to search for risk factors for acute optic neuritis recurrence.Furthermore, due to the lack of prospective clinical studies on recurrent acute optic neuritis,we are still uncertain whether there are clinical factors,in addition to glucocorticoid dosage, that affect acute optic neuritis recurrence. This study aims to make a cox regression analysis of ONTT date,to explore the risk factors of acute optic neuritis recurrent. Methods: The patients be grouped according to recurrence and non-recurrence.Kaplan-Meier and Cox regression analysis of the ONTT(a prospective randomized controlled trial)data set.And our models included effects of time, status, treatment groups,as well as demographic covariates of age, sex, race, visual acuity,et al.Results: The Cox regression analysis showed that the factors cause relatively high recurrence rates in oral prednisone group(HR:1.694,95% CI: 1.145-2.507,p=0.008)and age of onset <=30(HR:1.570,95% CI: 1.133-2.175,p=0.007).The data identified no relationship of sex, race,MRI result, contrast sensitivity, visual acuity outcomes, clinical center, optic disc edema, macular exudates, affected eye hemorrhage, family history of MS,history of neurological symptoms, viral syndrome in past 30 days or morbidity season. Conclusion: Age of onset <=30 and oral prednisone(1mg/kg prednisone/day)are important risk factors of acute optic neuritis recurrence.Clinical physician are suggested to treat acute optic neuritis patients without oral prednisone alone, which will increases the risk of new episodes of optic neuritis. And active follow-up of patients who younger than 30 years ago will have to be done to ensure that they benefit from treatment.
