| BackgroundAccording to the European Society of Cardiology statistics,worldwide adult coronary artery ischemia and chronic heart failure due to myocarditis caused by the accumulation of up to 26 million,annual new cases diagnosed with heart failure and about 2 million 600 thousand patients,one year mortality rate is as high as 17%-45%.Because of heart failure,many factors such as infection and other factors lead to acute attack,requiring repeated hospitalization,causing heavy medical burden to the society,and its treatment cost accounts for about 1%-2% of all medical expenditure.However,the treatment of heart failure is still a difficult problem.Many patients with heart failure have been treated with various means,but the effect is still unsatisfactory.Continuous hemofiltration is one of the most commonly used therapies in clinic.It is widely used in the treatment of refractory heart failure caused by various causes.The treatment of patients with severe edema with continuous stable dehydration and inflammatory cytokines treatment can significantly reduce the volume of fluid overload in the body,thereby improving diuretic resistance.However,some patients after ultrafiltration treatment will appear after fatigue,chills,pulmonary edema and pleural effusion caused by respiratory,urinary frequency and limb weakness and other symptoms,which belongs to the Chinese medicine Qi and yang deficiency two,if can give appropriate reviving Yang,tonifying qi and blood treatment,can promote the rapid recovery the physical condition of patients with heart failure after hemofiltration,symptoms improved with ease.From the "re Yan Jisheng" included Shenfu Decoction of Shenfu injection is due to the special effects are commonly used in clinical treatment,it can enhance the capacity and improve the heart function of heart contraction,increase heart output,improve blood pressure,regulate blood pressure play a role,but also by to reduce the effect of load,regulating blood pressure and protect myocardial cells of the heart before and after,many studies have confirmed that significantly better than simple treatment of Western medicine.ObjectiveTo inestigate the clinical efficacy of Shenfu Injection combined with continuoushemofitration therapy in treatment of patients with refractory HF.MethodsA total of 42 patients with refractory HF hospitalized in Henan provincial people's hospital from December,2015 to December,2016 were enrolled.These patients were randomly divided into control group and treatment group,21 cases in each group.All the patients were given conventional and continuous hemofitration therapy,and the treatment group were also treated with shenfu injection for 7 days while the control group were given placebo for the same time.The clinical effect,Minnesota Living with Heart Failure Questionnaire(MLHFQ)scores,body weight,heart rates,systolic pressure,total volume urine(in 24 hours),serum N-terminal pronatriuretic peptide(NT-ProBNP)level,left ventricular ejection fraction(LVEF)were observed before and after the treatment.Results1.After treatment,the clinical effect,body weight,heart rates,systolic pressure,total volume urine(in 24 hours)were all improved than before,and the improvements of clinical effect,the heart rates,systolic pressure were more significant in treatment group than control group while the improvements of the body weight and the total volume urine were no significant difference between two groups.2.The NT-pro BNP level was decreased significantly than before treatment,but the treatment group reduced more than the conrol group;The LVEF of both groups were increased after treatment,and compared with the control group,the treatment group were improved more significantly.3.The MLHFQ scores in the treatment group were lower than in the control group.ConclusionShenfu Injection combined with continuous hemofitration therapy could improve the cardiac function and the quality of life in patients with refractory heart failure. |
