Objective:To evaluate the usefulness of T-SPOT.TB test for the diagnosis of tuberculosis(TB)in the Chest hospital of Jangxi provinceMethods:We conducted a retrospective analysis of 599 patients who had a T-SPOT.TB test in Chest hospital of Jangxi province from Feb 2013 to Apr 2014.Divided into TB group and non tuberculosis, extrapulmonary tuberculosis and tuberculosis group, compared with the tuberculosis antibody, high sensitivity c-reactive protein, n/med tuberculosis bacterium smear tests, tuberculin test detection ability, and calculate the T-SPOT.TB in our degree of sensitivity, specific and positive predictive value and negative predictive value. The factors influence diagnostic value of T-spot.TB test was analyzed.Results:In thoracic specialized subject hospital, T cells spot test sensitivity and specific degree were 83.74% and 85.55% respectively. its positive and negative predictive value of test were 80.16% and 88.30% respectively. Tuberculosis and Non-tuberculosis group comparison, P=0.000.Between tuberculosis and Non-tuberculosis group, positive rate of T-SPOT was higher than the positive rate of TB and TB antibody, high sensitivity c-reactive protein, smear tests, tuberculin test 4 n/med tuberculosis bacterium detection method, compared the X2 value were 28.24(P<0.01), 5.93(P<0.05) and 8.39(P<0.01) and P=0.000, the detection positive rate differences have statistical value; The specific degree of T-SPOT was lower than it in pathology detection of TB, but higher than it in tuberculosis antibody, high sensitivity c-reactive protein and tuberculin test compared, X2 value were 27.50(P<0.01), P=0.000 and P=0.000, the difference has statistical value.In the pulmonary tuberculosis, positive rate of T-SPOT.TB was 88.00%, higher than that 73.24% of extrapulmonary tuberculosis, X2 =8.082(P<0.01), with statistical differences.Positive rate of T- SPOT. TB of pleural effusion was 94.34%, higher than 83.74% of peripheral blood of the TB(X2=5.70, P<0.05), the difference is statistically significant.Conclusions:T-spot.TB has high sensitivity and specific to the diagnosis of tuberculosis(TB), its diagnostic value is higher than the previous common tuberculosis antibody, high sensitivity c-reactive protein, n/med tuberculosis bacterium smear tests, such as tuberculin test method. The positive rate of T-spot of pulmonary tuberculosis was higher than that in extrapulmonary tuberculosis; The positive rate of T-spot of chest water was higher than that of peripheral blood. |
