Part ?Relativity between onset and lesion site of ischemic stroke patients caused by acute basilar artery occlusionObjective:Acute cerebral infarction caused by basilar artery occlusion induced complex clinical symptoms,signs,and a variety of forms.The purpose of this study is to explore the correlation between onset and lesion site of ischemic stroke patients caused by acute basilar artery occlusion.Methods:Fifty three patients diagnosed with cerebral infarction caused by acute basilar artery occlusion were divided into sudden onset group and progressive onset group.Their clinical and digital subtraction angiography data were analyzed retrospectively.Result:Middle and distal segment of the basilar artery occlusions were usually found in sudden onset group.Patients in progressive onset group were more likely to present with proximal segment of the basilar artery occlusions.There was significant statistical difference between two groups(P<0.05).Logistic regression analysis showed that the symptoms of patients with proximal segment basilar artery occlusion were tended to progressive onset,compared with patients affected by distal segment occlusion(OR=14.77,95%CI:1.57-139.00,P=0.019).Conclusion:There was a significant relationship between onset and lesion site of ischemic stroke patients caused by acute basilar artery oclusion.Early diagnosisand timely treatment may improve clinical outcome in these patients.Part ? Clinical Outcomes and Prognostic Factors Analysis of Endovascular Treatment in Ischemic Stroke Patients with Acute Basilar Artery OcclusionObjective:Acute basilar artery occlusion is a complex and severe diseases with the highest death rate and morbidity in ischemic stroke.The purpose of this study is to explore clinical outcomes and prognostic factors analysis of endovascular treatment in ischemic stroke patients with acute basilar artery occlusion.Methods:This study contains consecutive 53 patients diagnosed by basilar artery occlusion from January 2007 to July 2016 in Jinling hospital,and all patients underwent intravascular intervention therapy.All patients were followed up 3 months after onset according to the degree of self-care.The patients were divided into two groups by the modified Rankin scale(mRS):(1)favorable outcome(mRS<4),(2)poor outcome(mRS?4).Result:There was significant statistical difference between two groups(P<0.05).Logistic regression analysis showed that the clinical outcomes and prognostic factors analysis of endovascular treatment.After adjusted the NIHSS score,ASITN_SIR score and recanalization factors,favorable outcome was found in patients with the lower NIHSS score of admission(OR=0.114,95%CI:0.019-0.689,P=0.018),higher ASITN_SIR score(OR = 0.035,95%CI:0.005-0.239,P=0.001)and better recanalization(OR = 0.113,95%CI:0.017-0.767,P=0.026).Patients with lower ASITN_SIR,higher NIHSS score on admission or poor revascularization had a worse outcome.Conclusion:Patients with acute basilar artery occlusion had a better outcome in those who had lower NIHSS score on admission,higher ASITN SIR score and better recanalization.
