Objective:To observe the safety of endovascular interventional therapy for symptomatic atherosclerotic basilar stenosis in the perioperative period,and to analyze the risk factors of major complications.Methods:A retrospective analysis was performed on 57 patients who underwent endovascular treatment for basilar atherosclerotic stenosis in our hospital from January 2016 to December 2018,and the methods,success rate,perioperative complications and risk factors of endovascular treatment were observed.Results:Of the 57 enrolled patients,56 were successful in surgery,with a success rate of 98.2% and an incidence of perioperative complications of15.8%(9/57).Among them,5 patients had ischemic stroke,2 patients had TIA,1 patient had vascular dissection,1 patient had subarachnoid hemorrhage,and the rate of moderate or severe disability was 7.0%(4/57 m RS?3 points),with no death.During the follow-up period of 8.6±4.5months,3 of the 5 patients with ischemic stroke had better symptoms and 2 had worse prognosis.Among the 20 patients with diabetes,4 patients had ischemic stroke(20%),and 36 patients without diabetes,1 patient had ischemic stroke(2.7%),20%vs2.8%,P = 0.030,showing a significant difference.Conclusion:Endovascular intervention for symptomatic basilar atherosclerotic stenosis is feasible but still a high-risk procedure.Ischemic stroke caused by occlusion of perforating artery is a major complication of perioperative interventional treatment of atherosclerotic basilar stenosis,and patients should be fully evaluated before surgery,especially patients with diabetes mellitus,who may be at high risk of perioperative ischemic stroke.
