| Objective:Through the analysis of services in immunization units and children’s parents on the immunization service satisfaction and its influencing factors,and then explore the appropriate immunization service mode in Baoshan City.Methods:By using cross-sectional study,the information of all immunization units in Baoshan were collected by the China Immunization Program Information Management System.The stratified sampling method was used to select 815 parents from 45 villages of 15 townships in 5 counties for investigating their satisfaction with immunization services.The hardware facilities of the township immunization units and village immunization units in the townships and villages were investigated,and the ability of all the immunization staff in these immunization units were investigated too.The data were managed and analyzed by SPSS 20,and the statistical methods including chi-square test,Mann-Whitney U test,t test,Logistic regression and so on.Results:1.There are 992 immunization units in Baoshan City,township immunization units was 5.45%,village immunization units was 94.55%.Immunization units of the service form of fixed immunization,fixed + home immunization accounted for 90.16%,9.84%,township immunization units were all fixed immunization,village immunization units are still 10.41%fixed + home immunization.Fixed immunization,fixed + home immunization were covered by the city’s 90.52%,9.48%of the population.The daily and monthly immunization accounted for 48.80%and 51.20%respectively.The daily immunization was 94.23%in township units,and the daily and monthly immunization accounted for 46.18%and 53.82%respectively in village units.Township and village immunization units of the service population accounted for 13.83%and 86.17%of the total population of the city.The median of Immunization unit service population was 2238,of which the township unit was 1480,the village unit was 2254.The median of service radius was 6.0km,of which the township unit was 20.5km and the village unit was 5.0km.2.There are a total of 2441 immunization staff in Baoshan,of which the township unit was 6.76%and the village unit was 93.24.The average number of immunization personnel per 10,000 service population is 9.29,of which 4.54 are inoculated in the township immunization unit and 10.05 in the immunization unit.The median number of immunization staff per immunization unit was 2.0,of which he township unit was 2.5 and the village unit was 2.0.95.58%of the staff have practicing physician,practicing assistant physician,nurse or village doctor qualification certificate,98.89%of the staff have a immunization qualification certificate.Secondary education accounted for 70.46%,clinical professional accounted for 75.26%,still 4.14%of the staff for non-medical professional or no professional,82.14%of the staff without title.The enrollment of college students was 79.39%,and the village immunization staff were mainly secondary education(74.12%),college degree or above Accounting for 16.04%.Township staff medical specialty(including public health,clinical,nursing and other medicine)accounted for 98.79%,village staff of medical professional accounted for 95.65%,township staff with title(including early,)Accounted for 64.24%,village staff with titles accounted for only 14.50%.3.There are 815 parents.The proportion of parents who were satisfied with the convenience of the immunization unit,waiting time,immunization environment,service cycle,service attitude and technical level were 87.85%,69.69%,85.77%,64.66%,92.76%,87.48%.Village immunization units in the location of convenience,waiting time satisfaction is higher than the township units,and township units in the immunization environment,service cycle,technical level of satisfaction is higher than the village units.The total satisfaction rate of child parents was 68.96%,among which 6246%,70.15%of the township units and the village units were respectively.The influence factors of parents’ satisfaction for immunization service include vaccination dose,nationality,occupation,the time required go to immunization units,waiting time.4.A total of 115 inoculated persons were sampled.There was no difference in the level of vaccination between the township and the village,and the level of computer operation was higher than that of the village inoculation personnel.5.Sampling survey of 62 units.In the number of refrigerators,immunization room area,whether to vaccinate the special room,passbook printer equipment,township immunization unit is better than the village immunization unit.6.Survey data show that the township immunization unit match rate and measles vaccine with timely immunization rate is lower than the village inoculation unit.Conclusion:At present,the immunization service is mainly provided by village immunization units in Baoshan City.The most important immunization units for fixed immunization.The daily and monthly immunization close to each half.Immunization units hardware level is poor.Immunization staff education,professional level is generally low.The influence factors of parents’ satisfaction for immunization service include vaccination dose,nationality,occupation,the time required go to immunization units,waiting time,in Baoshan City.At present,Baoshan City is not yet have the conditions to fully implement township centralized immunization mode.Village immunization mode should continue to be maintained over a period of time. |
