Objective:Research and analysis the clinical and pathological parameters that may affect the non-sentinel lymph node(NSLN)metastasis in early-stage breast cancer,to analyze the non-sentinel lymph node status and the risk factors for NSLN metastasis in early breast cancer patients with one to two positive sentinel lymph nodes biopsy(SLNB)results.Methods:The clinical and pathological data of 94 breast cancer patients with one to two positive SLNB results admitted from January 2014 to December 2016 at the First Affiliated hospital of Guangxi Medical University were collected and divided into two groups based on the status of NSLN,one is the positive NSLN group(29 patients),the other is negative NSLN group(65 patients).The relations of the clinical and pathologic factors with NSLN metastasis were determined by univariate and multivariate Logistic regression analysis.Results:The metastatic NSLN was identified in 29 of the 94 patients with one to two positive sentinel lymph nodes(SLN)(30.8%).Univariate analysis showed that the ER,PR,histological grade,the tumor diameter,vessel cancer embolus,molecular subtyping and the number of positive SLNs were related to NSLN metastasis(all P<0.05);Multivariate Logistic regression analysis identified that vessel cancer embolus(OR=13.779,95%CI 3.244~58.534),the tumor diameter(OR=3.663,95%CI 1.080~12.224)and the histological grade(OR=3.331,95%CI 1.110~10.091)were independent risk factors for NSLN metastasis(all P<0.05).Conclusion:The ER,PR,histological grade,the tumor diameter,vessel cancer embolus molecular subtyping and the number of positive SLNs are related to NSLN metastasis.But the histological grade,the tumor diameter and vessel cancer embolus were independent risk factors for NSLN metastasis may be used as independent variables to predict NSLN metastasis in early-stage breast cancer patients with one to two positive sentinel lymph nodes from the SLNB result.The result of the study is accordance with international prospective studies about SLN,which may be used as reference index for clinical work.
