Objective: To explore the relationship between adiponectin receptor(Adipo R1,Adipo R2)and insulin receptor substrate 1(IRS-1)in endometrioid adenocarcinoma and its relationship with clinicopathological features and its clinical significance.Methods: The expression of adiponectin receptor and insulin receptor substrate 1 in 80 cases of endometrial adenocarcinoma,26 cases of atypical hyperplasia and 23 cases of normal endometrium were detected by immunohistochemical Max Vision method.And to analyze their relationship with clinicopathological parameters such as clinical stage and histopathological grade of endometrial adenocarcinoma.The statistical data were analyzed by chi-square test or Fisher's exact probability method.The correlation analysis was performed using Spearman rank correlation test.Results: Adipo R1 was expressed in endometrial adenocarcinoma,atypical hyperplasia and normal endometrium.The positive rates were 62.5%,73.1% and 91.3% respectively.The expression rate of endometrial carcinoma was lower than that of normal endometrium(P <0.05).There was no significant difference in the positive rate between endometrial carcinoma and atypical hyperplasia group.The positive expression rates of IRS-1 in endometrial adenocarcinoma,atypical hyperplasia and normal endometrial tissue were 72.5%,80.7% and 69.6%,respectively.There was no significant difference between them.In endometrial adenocarcinoma,the low expression of Adipo R1 is associated with FIGO staging,pathological grade,depth of myometrial invasion,lymph node metastasis,and lymphtic vascular invasion.The low expression of Adipo R2 was associated with histological differentiation and had no significant correlation with other clinicopathological features.The high expression of IRS-1 was associated with FIGO staging,pathological grade and lymph node metastasis.Correlation analysis showed that Adipo R1 was positively correlated with PR expression.Adipo R1 and Adipo R2 were negatively correlated with P53 expression,but had no significant correlation with the expression of ER and Ki-67.IRS-1 was positively correlated with the expression of P53 and Ki-67.Adipo R1 was positively correlated with Adipo R2 expression.Adipo R1 and Adipo R2 were negatively correlated with IRS-1 expression.Conclusion: The expression of Adiponectin receptor in the normal endometrium,atypical hyperplasia,endometrial adenocarcinoma tissue was decreased gradually,and IRS-1 expression was not significantly different.Adipo R was associated with the occurrence of endometrioid adenocarcinoma.The decreased expression of Adipo R and the increased expression of IRS-1 may be related to the occurrence,invasive and metastasis of endometrioid adenocarcinoma.Adipo R plays an inhibitory role in progression of endometrial adenocarcinoma,and insulin receptor substrate 1 may play a role in promoting it.The main effect may be achieved by Adipo R receptors,especially Adipo R1.Detection of Adipo R and IRS-1 expression is of great significance in predicting the biological behavior of tumors.
