BACKGROUND:Burns refers to damage of the skin,mucous membrane,and even deep tissue caused by heat,chemicals,electricity,radiation,in which thermal burns is the most common(such as hot water,fire,etc.)[1].Burns medicine after 50 years in China and the efforts of several generations,whether in the clinical diagnosis and treatment or in the basic theory researc as made remarkable achievement,over the years,the total cure rate,especially serious large area burn rate in remote area has ranked among the top of the world,which played an important role in a major catastrophic event of the year.In recent years,with the rapid development of social and economic construction,due to the era of industrialization of high energy injury caused by mass burn incidents continue to occur,the mass burn patients have characteristics of abruptness,relative concentration,quantity,complex injury,the patients have great difficulty in treatment.After the mass burn accident,it not only brought huge losses to the wounded and the accident unit,but also brought very huge social influence.OBJECTIVE:1?Retrospective analysis of the clinical data of mass burn cases caused byelectric bicycle fire in Nanning in recent two years,discuss the characteristics of mass burn patients and related factors affecting the prognosis,to summarize experience on treatment of patients to provide the clinical reference for further improving the cure rate of mass burn.2?In order to provide diagnosis and treatment flow for the mass burn patients in Nanning in the future.METHODS:To collect medical records of burned patients caused by electric vehicles fire in Nanning from January 1,2015 to December 31,2016,and to exclude those who have not been treated for financial or various reasons.The number of batches and statistics of the cause of the fire,timing,location,and each patient's gender,age,total burn surface area(%TBSA),third degree burn area,the incidence and prognosis of inhalation injury,bronchoscopic application,combined injury,shock,infection and visceral complications,summarize the treatment experience.RESULTS:1.In the 17 batch of 173 cases,14 batch occurred at night,3 batch occurred at daytime,14 batch occurred in the outside,6 batch occurred in the room,10 batch fire is due to the charging time is too long.Outpatient treatment of 108 people,hospitalized for treatment of 65 people,20-49 year old patients with high incidence,accounting for 61.62% of the total number of patients.The degree of burn was mainly mild and moderate,accounting for 73.58% of all cases,3 cases with lung blast injury,10 cases of soft tissue injury,2 cases of fracture,1 cases of craniocerebral injury.2.The degree of inhalation injury was mild and moderate,63 cases of mild accounted for 52.94%,39 cases of moderate accounted for 32.77%,17 cases ofsevere accounted for 14.29%.3.There were 9 patients died in this group,the mortality rate was 5.20%,including 5 males and 4 females.The main causes of death were inhalation injury in 7 cases(77.78%),severe craniocerebral injury in 1 cases(11.11%),combined injury with extensive burns in 1 cases(11.11%).4.Inhalation injury and severity,mechanical ventilation,fiberoptic bronchoscopy are related to prognosis.Inhalation injury is the main independent risk factor for the prognosis of mass burn patients.CONCLUSION:1.The mass burn accidents cause by electric bicycle fire mainly at night and indoors,It is not easy to be found until the smoke is inhaled.2.This group of patients in the confined space environment caused by inhalation injury,The treatment is more difficult compared with common skin burn;skin burns lighter,less than 10% of the burn surface area of patients accounted for69.94%.3.Inhalation injury is the main factor affecting the prognosis of mass burn patients caused by electric vehicle fires,and the mortality of patients with inhalation injury after airway lavage(endoscopic cleaning)can be obviously reduced.
