Objectives:This research aimed to study the changes of blood lipid and homocysteine(HCY)in patients with different malignant tumors,and explore its clinical value,and to analysis the association betweem abnormal lipid components with HCY and the occurrence of different types of malignant tumors.Methods:A clinical-based retrospect study was conducted in 1580 patients admitted to the internal medical department of the affiliated tumor hospit al of Guangxi Medical University,China.Among 1580 patients,there were 1245 malignant tumors as case group,335 benign lesions as control group.T he age,sex,height,weight,history,diagnosis and staging,serum total cholest erol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low d ensity lipoprotein cholesterol(LDL-C),apolipoprotein A1(APOA1),apolipoprote in B(APOB),lipoprotein a(LPa)of all grouped cases were collected.We ai med to study the effect of different types of malignant tumors on blood l ipid levels and its clinical value through statistical analysis,to speculate th e prognostic value of increased HCY to the diagnosis of malignant tumor s and predict prognosis through ROC curve analysis,to study the associat ion betweem lipid and HCY and malignant tumors.Results:1.Different types of malignant tumor patients with differnt lipid level compare to benign lesions.HDL-C:liver cancer(1.07±0.42)mmol/L,lung cancer(1.16±0.30)mmol/L?digestive tract malignant tumor(1.17±0.31)mmol/L were higher than benign lesion(1.32±0.37)mmol/L,(P<0.05);LDL-C:liver cancer(4.01±1.85)mmol/L,digestive tract malignant tumor(3.97±1.25)mmol/L were higher than benign lesion(3.73±1.35)mmol/L,(P<0.05);TG:liver cancer(1.26±0.69)mmol/L,digestive tract malignant tumor(1.35±0.83)mmol/L lower than benign lesion(1.52±1.16)mmol/L,(P<0.05);APOA1:liver cancer(0.96±0.33)g/L,lung cancer(1.09±0.26)g/L,digestive tract malignant tumor(1.08±0.26)g/L were lower than benign lesion(1.22±0.31)g/L,(P<0.05);While breast cancer and gynecological malignancies,serum lipids were not statistically significant compare to benign lesions(P>0.1).2.Different types of malignant tumors had different blood lipid level in different stages.The stage were more earlier,the HDL-C level were lower in breast cancer,liver cancer,digestive tract malignant tumor,gynecology malignant tumor.The stage were more later,the TG level were higher in liver cancer,digestive tract malignant tumor,and the HCY level were higher in liver cancer,digestive tract malignant tumor,gynecology malignant tumor.3.There were significant differences in serum HCY levels between benign and malignant tumors.Liver cancer(16.38±4.11)?mol/L,lung cancer(15.53±3.73)?mol/L,digestive tract malignant tumor(15.11±3.57)?mol/L were higher than benign lesion(14.29±3.59)?mol/L,(P<0.05).The level of H CY in male patients with malignant tumor was significantly higher than t hat in female patients(t=11.963,P=0.000).4.The increased HCY have prognostic value to the diagnosis of malignant tumors and predict prognosis through ROC curve analysis.Liver cancer,s AUC was 0.666,Sensitivity 72.2%,specificity54.3%.The optimal cutoff of HCY is14.15?mol/L.5.Logistic regression analysis showed that there was a negative correlation between the incidence of liver cancer and LDL-C,TG and LPa,and positively correlated with HCY and APO-B.There was a negative correlation between the incidence of lung cancer and LDL-C level,and positively correlated with HCY and LPa levels.There was a negative correlation between HDL-C,APO-A1 level and BMI in digestive tract malignancy,and positively correlated with LDL-C and HCY.Conclusions:1.Malignant tumor had lipid metabolism disorders.Abnormal dyslipidemia may be a risk factor for malignancy.Abnormal blood lipid levels may be related to the severity of the disease for liver cancer,gastrointestinal cancer,breast cancer patients.2.Liver cancer,lung cancer,gastrointestinal cancer patients had elevated serum HCY significantly.Serum HCY levels as a tumor marker to diagnose the disease had a certain clinical value.3.Serum HDL-C,TG,LPa is a protective factor for hepatocellular carcinoma,while HCY,APO-B is a risk factor.HDL-C is a protective factor for lung cancer,while HCY,LPa is a risk factor.HDL-C,APO-A1 is a protective factor for digestive tract malignancies,while LDL-C,HCY is a risk factor.So,regular detection of blood lipid levels in the prevention of liver cancer,lung cancer,gastrointestinal cancer have a certain guiding significance.
