Objective:The model of cerebral infarction complicated with bacterial pneumonia was established by 3 kinds of different bacterial inoculation methods,which were combined with nasal dropping method,oral endotracheal intubation and tracheal injection.To select the best method to establish the rat model,and observe the peripheral white blood cell count,neutrophile granulocyte,pathological indexes and changes in intestinal flora,and the impact ofQing Re Hua Tan Tong Fu Decoct on these above indexes.To preliminarily investigate the possible mechanisms of Qing Re Hua Tan Tong Fu Decoct in the treatment of cerebral infarction with bacterial pneumonia.Method: 1.76 SPF grade male SD rats,all were established by using the modified thread embolism method.After they were valuated by Longa 5 grades,randomly selected 4 rats with cerebral infarction killed and removed brain tissue to TTC staining.The other 72 rats were randomly divided into intranasal group,oral endotracheal intubation group,trachea injection group and cerebral infarction control group,each group had 18 rats.The four groups according to the treatment time were divided into ld,3d,7d those three subgroups,each subgroup had 6 rats.The three model group on the basis of the model of cerebral infarction combining nasal drip,oral endotracheal,trachea injection to establish the model of cerebral infarction complicated with bacterial pneumonia by inoculating of Pseudomonas aeruginosa 0.2ml bacterium suspension.Rats in the control group after the establishment of cerebral infarction model are without any other processing.We observed the general physiological status,body temperature and body weight measurement,detection of peripheral white blood cell count and neutrophil percentage,and combined lung tissue of rats of bacteriological and pathological examination. To compare and analysis the change of rat above index of three kinds of modeling methods and evaluate these models.2.30 SPF SD male rats were divided into 5 groups according to the random number table: the normal group,model group,high dose of Chinese medicine enema group(High dose group),medium dose of traditional Chinese medicine enema group (Medium dose group),low dose of Chinese medicine enema group(Low dose Group),and each group had 6 rats.To select the best method to establish the rat model with cerebral infarction complicated with bacterial pneumonia.The high dose group,medium dose group and low dose Group were respectively to high,medium and low doses of enema in the treatment with Qing Re Hua Tan Tong Fu Decoct.No treatments for model group and normal group.After 7 days of treatment,observation of rat general state,weight and body temperature changes;Through automatic blood cell analyzer to detect rat peripheral white blood cell count and neutrophil percentage of cells;HE staining was used to observe the changes of the rats lung tissue pathology;And the bacteriological detecte of rats intestinal micro ecological changes.Results:1.Modified thread embolism method were combined with three methods to establish cerebral infarction with bacterial pneumonia rat model. Each group was in varying degrees with changes in every indicators:In the general performance of the state and the severity of elevated temperature,the oral endotracheal intubation group and the tracheal injection group were poor in general condition,and their severity is higher than intranasal group.In the reduction of body weight,the weight of tracheal injection group decreased most significantly.In the aspect of peripheral white blood cell count and neutrophil percentage and duration,all three groups were significantly increased.In the success rate of modeling,the tracheal injection group had the highest success rate of modeling.Comprehensive evaluation:the severity of the indexes of the tracheal injection group was higher,and the highest success rate of modeling was the highest,so this model was the best.2.Comparing with the normal group,rats in the model group were all in poor general condition,body weight decreased significantly (P<0.01),body temperature,peripheral white blood cell count and neutrophil cell percentage increased significantly (P<0.01).Comparing with model group,rats in high dose group were all in good general condition,body weight increased significantly (P<0.01),peripheral white blood cell count and neutrophil cell percentage decreased significantly (P<0.01),no similar differences were found between normal group (P>0.05).Rats in medium dose group and low dose group were in good condition,and their weight increased obviously(P<0.01),and their body temperature,peripheral white blood cell count and neutrophil cell percentage all dropped (P<0.05), but still higher than normal group(P<0.05).3.In model group,the pulmonary edema and congestion were obvious,and the alveolar stroma was increased,with a large number of inflammatory cell infiltration.Rats in high dose group,the lung tissue edema and congestion were not significant,alveolar interstitial matter were not broadening,and without inflammatory cell infiltration. Rats in medium group and low group,their lung tissue edema and congestion were significant,the alveolar between matter were slightly broadening,with amount of inflammatory cell infiltration.4.Lung tissue homogenate of normal group rats can not cultivate pseudomonas aeruginosa;lung tissue homogenate of model group rats can pseudomonas aeruginosa. Compared with model group,pseudomonas aeruginosa number of high dose group was reduced,the difference was statistically significant(P<0.05);pseudomonas aeruginosa number of medium dose group and low dose group of were reduced,but there difference were no statistical significance(P>0.05).5.Comparing with the normal group,the intestinal aerobic bacteria,anaerobic bacteria,Escherichia coli and the bacteria in the model group were significantly increased (P<0.05 or P<0.01),and intestinal lactic acid bacteria was significantly reduced (P<0.05).Comparing with the model group,rats in high dose group's naerobic bacteria and Escherichia coli decreased significantly(P<0.05)and Lactobacillus increased significantly (P<0.05).Rats in medium group and low group's Lactobacillus increased,and the other intestinal flora were without significant difference (P>0.05).Conclusion: 1.Modified thread embolism method combined with intratra-cheal injection established cerebral infarction with bacterial pneumonia rat model the success rate is highest,easy to operate and well repeatability.2.The rat model with cerebral infarction complicated with bacterial pneumonia had the dysbiosis of intestinal flora.3.Qing Re Hua Tan Tong Fu Decoct enema in the treatment of cerebral infarction with bacterial pneumonia rats can improve the pneumonia and dysbiosis of intestinal flora.The high dose administered treatment effect is better than others.Qing Re Hua Tan Tong Fu Decoct enema treatment of one of the mechanism of the cerebral infarction with bacterial pneumonia in rats may be related to the improvement of rats's dysbiosis of intestinal flora.
