| Objective:To observe the clinical efficacy of Shenfu injection in the treatment of acute exacerbation of chronic heart failure in patients with heart and kidney yang deficiency type.Methods:Choose forty hospitalized patients who belong to acute exacerbation of chronic heart failure(NYHA grade ? or grade ?)with heart and kidney yang deficiency type,in the Department of Cardiology,the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from January 2016 to December 2016.These patients were randomly divided into two groups:the treatment group and control group,20 cases each.Both groups were given the standard treatment of heart failure in Western medicine.On the basis of these,the treatment group were treated with intravenous infusion of Shenfu injection 50 ml,diluted with 5% glucose 100 m L once a day.The course of treatment was two weeks.No other Chinese medicine preparations were used in both groups.Then,compared two groups of patients with Chinese medicine syndrome effect,Minnesota Quality of Life Score,NYHA cardiac function,B-type Natriuretic Peptide,Left ventricular ejection fraction,Left ventricular end-diastolic diameter,uric acid,bilirubin and other indicators before and after treatment.Results:1.Curative Effect of TCM Syndrome: After treatment,the total effective rate of the treatment group was 90.0%,the total effective rate of the control group was 65.0%.The results of the two groups were significant by statistical analysis(P<0.05).2.Comparison of TCM syndrome points,Individual symptoms of TCM and Minnesota Quality of Life Score:(1)Comparison in the two groups before and after treatment:In the two groups,the TCM syndrome points,Individual symptoms of TCM and Minnesota quality of life scores after treatment were different compared with thesame group before treatment(P<0.05 or P<0.01).(2)Comparison between the two groups after treatment:Compared with the control group,the treatment group improved more obviously after treatment(P<0.05).3.The effect of heart function:After treatment,the total effective rate of the treatment group was 90.0%,the total effective rate of the control group was 75.0%.The results of the two groups were significant by statistical analysis(P<0.05).4.Comparison of B-type Natriuretic Peptide,Left ventricular ejection fraction values:(1)Comparison in the two groups before and after treatment:After treatment,the two groups of BNP compared with the same group before treatment were different(P<0.05),the treatment group of LVEF compared with the same group before treatment were different(P<0.05).But there was no significant difference of LVEF in the control group before and after treatment(P>0.05).(2)Comparison between the two groups after treatment:The BNP and LVEF of the treatment group were superior to the control group after treatment(P<0.05).5.Comparison of Left ventricular end-diastolic diameter values:There were no difference of LVDd compared with the same group before and after treatment in the two groups(P>0.05).Comparison between the two groups after treatment also did not make sense(P>0.05).6.Comparison of uric acid,bilirubin values:(1)Comparison in the two groups before and after treatment:In the two groups,the uric acid,total bilirubin,direct bilirubin and indirect bilirubin after treatment were different compared with the same group before treatment(P<0.05).(2)Comparison between the two groups after treatment:Compared with the control group,the treatment group was better in terms of uric acid,total bilirubin,direct bilirubin and indirect bilirubin values(P<0.05).7.Comparison of 24 hours urine output:(1)Comparison in the two groups before and after treatment:The 24-hour urine output of the two groups on day 1,day 3,day 7and day 14 after treatment were different from that of the same group before treatment(P<0.05).And in the two groups after treatment,the 24-hour urine output ofthe third day was different with the same group on the first day(P<0.05).The 24-hour urine output of the 7th day was different with the same group on the third day(P<0.05).But there was no difference between the 14 th day and the 7th day of the24-hour urine output in the same group(P>0.05).(2)Comparison between the two groups after treatment: Compared with the control group,the 24-hour urine output of the treatment group of patients on the first day,the third day,the 7th day and the 14 th day after treatment were more significant(P<0.05).8.Observation of adverse reactions: During the observation,only one person in the treatment group appeared facial redness and facial fever phenomenon,while the monitoring of temperature was normal.Two people had temporary dizziness in the control group.The above patients got better with no special treatment.The remaining patients did not appear obvious or serious adverse reactions.Conclusion:Shenfu injection combined with conventional anti-heart failure of Western medicine in the treatment of acute exacerbation of chronic heart failure(NYHA grade? or grade ?)in patients with heart and kidney yang deficiency type has a better effect,in improving the efficacy of TCM syndrome,quality of life score,NYHA cardiac function,B-type Natriuretic Peptide,Left ventricular ejection fraction,blood Uric acid,serum bilirubin levels and 24-hour urine output,compared with the simple application of conventional anti-heart failure of western medicine to treat heart failure. |
