Effect Of Rong Huang Tonify Kidney Detoxification Granules On The Serum BMP-7?1,25?OH?₂D₃ Of Non Dialysis CKD-MBD Patients With Kidney Deficiency Damp Heat Syndrome

Objective In this study,we mainly through observing the changes of serum 1,25-dihydroxyvitaminD3?1,25?OH?₂D₃?,renal bone morphogenetic protein-7?BMP-7?numerical value in non-dialysis chronic kidney disease mineral and bone metabolism disorder?CKD-MBD?which patients who with kidney deficiency and damp heat,to evaluated the therapeutic effect and reseach possiblely mechanism about Rong Huang tonify kidney detoxification granules.Methods Choose 66 patients of CKD-MBD with kidney deficiency and damp heat TCM,both of them are non-dialysis,met the requirements of inclusion criteria.Divided the patients into two groups,treatment group and control group-A,randomly,each of them has 33 cases.Actually,60 patients are completed.Meanwhile,set up a group with 20 cases who are in chronic kidney disease 1 period.Treatment group and control-A group patients were treated by corresponding therapy,such as redcuce blood preasure,correct renal anemia,acidosis and traditional Chinese medicine oral and enema,etc.Besides,Rong Huang tonify kidney detoxification granules were added in treatment group.The observation period of study was 8 weeks.During the study,observe and record the efficacy of disease,curative syndrome,mian symptoms,and biochemical indexes,such as,serum calcium?Ca?,phosphorus?P?,alkaline phosphatase?ALP?,parathyroid hormone?iPTH?,each index was detect three times,before,duiring and after treatment.Meanwhile,the levels of left femur and vertebral bone?L1-4?bone mineral density?BMD?and the serum 1,25?OH?₂D₃,BMP-7 were detect,each patients were detect two times,before and after treatment.According to the results of the study,using statistical software to analysis the efficacy of disease.Results After eight weeks' treatment,the effect of treatment group were better than the control group-A in the efficacy of disease,curative syndrome,mian symptoms,and the difference were significant?P<0.05 or P<0.01?.At the end of fourth and eighth week,BUN in treatment group was decreased than before,the difference was significant in statistic?P<0.05 or P<0.01?;At the end of fourth week,Scr was decresased and the eGFR was incressed,the difference were significant?P<.05?;At the end of eighth week,only BUN decreased,the difference were significant?P<0.05?,and all indexes in treatment group were better than control-Agroup.At the end of eighth week,the treatment group,the serum Ca was increased,the serum P,iPTH,ALP were decreased,the dates were significant?P<0.05 or P<0.01?;In control group-A,serum P,iPTH were significantly decreased?P<0.01?,those indexes in treatment group were significant compared with control-A group?P<0.05?.The levels of left femur and vertebral bone?L1-4?bone mineral density?BMD?in treatment and control-A group were lower than control-B group,the dates were remarkablely significant?P<0.01?.At the end of treatment,left femur and L1-4BMD were increased,the dates were significant?P<0.05?;But,in control group-A,all of them were no significant?P>0.05?.The serum 1,25?OH?₂D₃ and BMP-7 were lower than control-B,the dates were significant?P<0.05?.At the end of eighth week,the dates of serum 1,25?OH?₂D₃ and BMP-7 were incereased in treatment group?P<0.05 or P<0.01?,but in control-A group,all of them were no significant?P>0.05?.According to Pearson method,to analyze indexes,serum 1,25?OH?₂D₃,BMD are positive correlation with eGFR?P<0.01?,but were negative correlation with serum BUN,Scr,P,iPTH,and the differences also significant?P<0.01?.During the treatment,both treatment group and control-A group were no adverse reactions.Conclusions Most patients with CKD-MBD and the kidney deficiency and damp heat in non dialysis,the serum of 1,25?OH?₂D₃,BMP-7 and BMD and T score were lower than CKD1 period patients.This indicate that from CKD3 period,patients can occur abnormal bone metabolism.With the cure of Rong Huang tonify kidney detoxification granules,the drugs were beneficial for clinical symptoms and renal function,the serum Ca was increased,and the serum P,iPTH,ALP were decressed,patients can achieve the purpose of preventing and control.The mechanism may be related with the improving.of bone matebolism?BMP-7,1,25?OH?₂D₃?.
