Effect Of Rong Huang Granules On The Serum BALP?TRAP Of Non Dialysis CKD-MBD Patients With Kindney Deficiency Damp Heat Syndrome

Objective: The article observes the changes of bone-specific alkaline phosphatase(bonealk alinephos phatase,BALP)and serum tartrate-resistant acid phosphatase(tartrate resistantant acid phosphatase,TRAP)levels in the kidney deficiency and damp-heat syndrome of patients with non-dialysis chronic kidney disease mineraland bone disorder metabolism(chronic kidney disease mineraland bone disorder,CKD-MBD)and the changes of related indicators after treatment with Ronghuang granule,to discuss possible mechanism and the therapeutic effect of Ronghuang Granule on CKD-MBD.Methods: From December 2017 to October 2018,70 patients with non-dialysis CKD-MBD kidney deficiency damp-heat syndrome were enrolled in the Department of Nephrology,Anhui Provincial Hospital of Traditional Chinese Medicine,and were randomly divided into control group and treatment group.During the treatment,the control group and the treatment group were separated from 5 cases and 4 cases respectively.Another 20 healthy subjects were selected as the normal group.Both the control group and the treatment group were given the corresponding hypotension,correcting renal anemia,correcting disorders of calcium and phosphorus metabolism and other symptomatic treatment.At the same time,two groups of patients were given enema therapy with Chinese medicine.treatment group on this basis plus Ronghuang granules(three times a day,one pack each time)treatment.Two groups of patients treated for 12 weeks.In the process of research,the changes of TCM syndromes integral value of two groups were recorded;serum creatinine(serum creatinine,Scr),urea nitrogen(blood urea nitrogen,BUN),serum phosphorus(phosphorus,P),serum calcium(calcium,Ca),parathyroid hormone(intact parathyroid hormone,iPTH),BALP and TRAP were also detected before and after treatment in two groups,and only serum BALP and TRAP were detected in normal group.According to the results of the experiment,the therapeutic effects of CKD-MBD,renal function,TCM syndromes and related biochemical indicators were determined by SPASS21.0 software.Results: The curative effect of CKD-MBD in the treatment group was significantly better than that in the control group(P < 0.05);the integral value of TCM syndromes in the control group and the treatment group decreased more obviously with the increase of the course of treatment(P < 0.05 or P < 0.01),but the decreasing extent of CKD-MBD in the treatment group was significantly better than that in the control group(P < 0.05 or P < 0.01).After treatment,Scr,BUN,eGFR,Ca,P and iPTH in the treatment group were significantly improved at 4,8 and 12 weekends(P < 0.05 or P < 0.01);Scr,BUN,eGFR,Ca,P and iPTH in the control group were significantly improved at 12 weekends(P <0.05);Scr,BUN,eGFR,Ca,P and iPTH in the treatment group were significantly better than those in the control group at 8 and 12 weekends(P < 0.05 or P < 0.01).Before treatment,the levels of BALP and TRAP in the control group and the treatment group were significantly higher than the levels of BALP and TRAP in the normal group(P<0.01)and there were no obvious discrepancy in serum BALP and TRAP levels between the control group and the treatment group(P > 0.05).After treatment,the levels of BALP and TRAP in the treatment group were significantly lower than those before treatment(P < 0.05),while the levels of BALP and TRAP in the control group were not significantly different from those before treatment(P > 0.05);the levels of BALP and TRAP in the treatment group were significantly lower than those in the control group at the same time(P < 0.05).Conclusion: Ronghuang Granule can significantly improve the curative effect,renal function,TCM syndromes and laboratory examination indexes of non-dialysis CKD-MBD patients with the syndrome of dampness-heat and kidney deficiency.The levels of serum BALP and TRAP dampness-heat and inkidney deficiency syndrome of non-dialysis CKD-MBD patients were obviously higher than those in normal people.Ronghuang granule can effectively reduce the levels of BALP and TRAP in non-dialysis CKD-MBD patients with the syndrome of dampness-heat and kidney deficiency.This may be one of the mechanisms of Ronghuang granule in improving clinical symptoms,renal function,elevating blood Ca level,reducing blood P and iPTH levels in non-dialysis CKD-MBD patients who have the syndrome of dampness-heat and kidney deficiency.
