Craniofacial Fascia Multipoint Fixed Forming Method In Correcting Cupped Ear Malformation

Objective: To introduce the application of craniofacial fascia multipoint fixed forming method in correcting cupped ear malformation,and to explore the therapeutic effect of the surgery through the assessment of auricle subunit structure exposure,auricle symmetry and postoperative satisfaction.Methods: 39 cases of patients with unilateral cupped ear malformation(Tanzer type I and II)who received treatment in the Plastic Surgery of Sichuan Academy of Medical Sciences & Sichuan Provincial People's hospital from January 2015 to March 2016 were selected as the research object.Preoperative assessment was conducted on their auricle subunit structure exposure and auricle symmetry.Cranial sulcus incision,was selected in the surgery,and fully exposed.The retroauricular cartilage was released;the auricle cartilage was adjusted upwards and backwards and by extorsion to satisfactory position.The defected or malformed auricle structure was restored according to the situation of individual patients.Finally,the three parts,namely,the triangle fossa uplift,outer rim of fossae helicis and auricular concha uplift were respectively fixed on the cranial side deep fascia and skull periosteum.After stooping bleeding and placing drainage strip,the incision was sutured to complete surgery.Follow-up was conducted 7 days and 1 year after the surgery.Moreover,the postoperative auricle subunit structure exposure,and auricle symmetry were evaluated.The statistical analysis was conducted on the results through SPSS21.0.Results:53 cases of patients successfully completed surgery,and 51 cases of patients had no related complications after surgery.2 cases of patients had subcutaneous hematoma,and discharged from the hospital after healing through treatment.The surgical effect was evaluated the follow-up results 3 month and 1 year after the surgery.The following results were obtained through the statistical analysis: Firstly,preoperative and postoperative auricle subunit structure exposure showed differences,and p<0.001,showing the differences were statistically significant.For the comparison of postoperative result of 3 month and 1 year,p>0.05,and there were no statistical differences.Secondly,preoperative and postoperative auricle symmetry showed differences,and p<0.001,showing the differences were statistically significant.For the comparison of postoperative result of 3 month and 1 year,p>0.05,showing there were no statistical differences.According to the satisfaction survey results of the surgical effect,the majority of patients,and their families and surgeons were satisfied with the surgical effect.Conclusion: Firstly,craniofacial fascia multipoint fixed forming method can effectively treat type I,type II cupped ear malformation.Secondly,craniofacial fascia multipoint fixed forming method has stable auricle form after surgery in treating type I,type II cupped ear malformation and the patients and families have high satisfaction at it..Thirdly,Craniofacial fascia multi-point fixed treatment of type I,type II cup ear deformity of the case is the upper part of the ear is not the amount of tissue defects,deformities and ear upper part of the ear with the abnormal development of the Ministry of the ear.Fourthly,The long-term effect of craniofacial fascia multipoint fixed forming in curing type I,type II cupped ear malformation still needs further follow-up study.
