A Study: Adiponectin Expression Level In Benign And Malignant Breast Lesion And Para-lesion Tissues

Objective:This study was to detect the adiponectin concentration of lesion tissue,para-lesion gland tissue,para-lesion adipose tissue in benign and malignant breast disease.It aimed to compare whether there were differences ahout the expression level of adiponectin among different tissues,and to explore the relationdship between tissue adiponectin and the relevant parameters of breast cancer occurrence and prognosis judgement.Which kind of role was played in the occurrence and development of breast cancer by tissue adiponectin was further analysed.It was hoped to find out new markers for the clinical guidance of breast cancer diagnosis and prognosis judgement.Methods :Patients with breast mass diagnosed at the first time in our department from March 2016 to September 2016 were enrolled.They were assigned into the breast cancer group(A group)and the breast benign disease group(B group)according to the postoperative pathological results.Each patient's fasting venous blood(A??L?),lesion tissue(A??L?),para-lesion gland tissue(A??L?),para-lesion adipose tissue(A??L?),as well as the relevant clinical and pathological parameters were collected.After the venous blood was centrifuged,the upper serum was extracted,and the supernatant was saved after tissue homogenate.The concentration of adiponectin in serum and supernatant were measured by ELISA(enzyme-linked immunosorbent assay).Statistical analysis:(1)The comparison in group: in A group,the tissue adiponectin concentrations among A?,A?,A?were compared to study whether there were differences.In L group,the tissue adiponectin concentrations among L??L??L?were compared,too.(2)The comparison between groups: whether there were significant differences about the adiponectin concentration between A? and L?,A?and L?,A?and L?,A?and L?were respectively analysed.(3)In A and L group,the adiponectin expression level was respectively compared according to the different clinical characteristics(age,menstruation,exercise,fertility number,BMI,etc.),at the same time,the A group was grouped by different pathological characteristics(TNM stage,Luminal molecular classification,WHO classification,lymph node metastasis,distant metastasis,ER,PR,HER-2,Ki-67 etc.)in order to compare if there were differences between them.(4)Correlation analysis: in A and L group,the respectively correlation between A?,A?,A?,A?,L?,L?,L?and the above clinical and pathological parameters were simply explored in this paper.The unified test level : ?=0.05.Results:(1)Common characteristics of patients:There were 41 patients in A group,with ages ranging from 26 to 77,among these patients,20 cases were postmenopause,and 21 cases were premenopause.There were 35 cases of invasive ductal carcinoma,3 cases of invasive lobular carcinoma,1 case of endocrine carcinoma,1 case of mucinous carcinoma and 1case of papillary carcinoma.The L group enrolled 38 patients,with ages ranging from 26 to 76,among them,20 cases were postmenopause,and 18 cases were premenopause.There were 24 cases of fibroadenoma,8 cases of phyllodes tumor,6 cases of adenosis of breast.(2)Comparison of adiponectin level in tissues:The result of ANOVA analysis in A group showed: F was 5.042,p was 0.008,that was to say: there were differences among the three tissues.The tissue adiponectin concentration of A?was significantly lower than that of A?(2783.94±1781.54 pg/ml vs 3902.85±1971.91 pg/ml,p=0.010)andA?(2783.94±1781.54 pg/ml vs 4016.25±2062.76 pg/ml,p=0.005),respectively.But there was no significant difference between A?and A?(3902.85±1971.91 pg/ml vs 4016.25±2062.76 pg/ml,p= 0.792,p was greater than 0.05).The ANOVA analysis result in L group: F was 0.373,p was 0.689,in the other word: the level of adiponectin in the three groups of L?(4238.19±1626.76 pg/ml),L?(3911.72±1913.21 pg/ml),L?(4006.97±1516.70 pg/ml)were not statistically significant.(3)Comparison of adiponectin level between groups:The adiponectin concentration of A ?(5580.35±1823.34 pg/ml)was significantly lower than that of L ?(6918.90±1137.76 pg/ml),p was 0.000.When the tissue adiponectin level of A?(2783.94±1781.54 pg/ml)compared with L?(4238.19±1626.76 pg/ml),the p was 0.000,so there was significant statistical difference.While the comparation of A?and L?(3902.85±1971.91pg/ml vs 3911.72±1913.21 pg/ml,p=0.984),as well as the comparation of A?and L?(4016.25±2062.76 pg/ml vs 4006.97±1516.70 pg/ml,p=0.982)showed no significant difference.(4)Grouped by clinical and pathological characteristics:The analysis of A group showed that tissue adiponectin concentrations of A ?(p=0.001),A?(p=0.048),A?(p=0.004),A?(p=0.023)in the Ki-67 low expression group was higher than that in the Ki-67 high expression group,the difference was statistically significant.There was no significant difference which was observed in L group.(5)Correlation analysis:In breast cancer group,the expression level of Ki-67 was negatively correlated with the adiponectin concentration of A ?(r=-0.537),A?(r=-0.420),A?(r=-0.544),A?(r=-0.386).The adiponectin level of A ? was positively correlated with that of A?(r=0.489),A?(r=0.657),A?(r=0.603).But the BMI(r=-0.420)and the weight(r=-0.507)were negatively correlated with theadiponectin level of A ?.About the adiponectin expression level,A?was positively related to A?(r=0.697),A?(r=0.636).A?and A?(r=0.772)was found to be positively correlated.Conclusions:We get conclusions from this prospective study:(1)The adiponectin concentration of A? and A?in patients with breast cancer are lower than that in benign breast patients.It is specificity.So serum adiponectin and tissue adiponectin are helpful to identify the breast mass.(2)There is some correlation between adiponectin(in blood or in tissue)and proliferation activity(Ki-67)of breast cancer cells.
