Clinical Analysis Of 471 Patients With Benign Paroxysmal Positional Vertigo In Neurology Department

Objective:Through the clinical analysis of the patients with benign paroxysmal positional vertigo(BPPV)in the neurology clinic,the clinical features were summarized to reduce the misdiagnosis and misdiagnosis of BPPV.Methods:Collected from January 2016 to June 2016,471 patients with BPPV were enrolled in the Department of Neurology,the First Affiliated Hospital of Jilin University,such as gender,age,time duration,seizure frequency,dizziness,vertigo and relief incentives from the treatment time,accompanying symptoms,neurological examination,auxiliary examination,diagnosis,treatment and therapeutic effect of reduction and other indicators,analysis of clinical patient data collection,and data statistics processing.Results:(1)A total of 1289 patients with vertigo were examined in No.1Hospital of Jilin University,among which 471(36.54%)were diagnosed as BPPV.(2)In the patients with BPPV,the ratio of male to female was1:2.(3)The youngest BPPV patients was 15 years old,the oldest patient was 82 years old,the average age was 53.95 years old,the incidence wasmainly distributed in 40-69 years,in patients with BPPV in the age difference was statistically significant(P < 0.05).(4)Dizziness,family history,hypertension,hyperlipidemia,otitis media,history of head injury and BPPV was relatively independent,there was no statistically significant difference(P > 0.05),the incidence of diabetes was statistically significant in BPPV(P < 0.05).(5)A total of 348 cases of posterior semicircular canal BPPV(73.89%);anterior semicircular canal BPPV a total of 10 cases(2.12%);the horizontal semicircular canal BPPV a total of 92 cases(19.53%);hybrid BPPV a total of 21 cases(4.46%).(6)Look up,down,up,down position can be induced in 310 patients and PC-BPPV in patients with exacerbation of vertigo(89.08%);HC-BPPV patients with left turn and right turn(or)can be induced and aggravated in 71 cases of vertigo(77.17%);SC-BPPV patients rise,bow,and vertigo induced exacerbations get up,lay down position in 5 cases(50.00%).(7)With visual rotation in 416 cases(88.32%);a sense of dizziness,ignoring the rotating 31 cases(6.58%);their sense of shaking 8cases(1.70%);with nausea and vomiting,sweating 274 cases(58.17%).The difference of accompanying symptoms was statistically significant in BPPV(P < 0.05).(8)The time last seconds to 1 minutes in patients with vertigo was 423 cases(89.81%);for 1 to 10 minutes in 31 cases(6.58%);10 minutes to 1 hours in 7 cases(1.49%);lasting more than 1 hours in 10patients(2.12%).The duration of vertigo was statistically significant inBPPV(P < 0.05).(9)In this study,given the vertigo drug treatment(including oral drugs and intravenous drug)in patients diagnosed 233 cases of BPPV(33.72%),the diagnosis of patients with vertigo drug treatment did not give for 238 cases of BPPV(39.80%).The effect of the treatment of vertigo on the diagnosis of BPPV was statistically significant(P < 0.05).(10)BPPV patients were treated with reduction,and the total effective rate(including recovery and improvement)was about 94.06%.(11)The length of time from the onset of treatment had statistical significance on the effect of treatment.Among them,the total effective rate was the highest(56.21%)in patients who were less than 1 days after the treatment.Conclusion:In patients with BPPV,the number of male patients was less than the number of female patients.The patients aged from 40 to 69 years were the high risk group.For various types of BPPV patients in the proportion from high to low is: PC-BPPV,HC-BPPV,BPPV of mixed canal type,SC-BPPV.The clinical manifestations of patients with BPPV were mainly related to the position of the object rotation,less than a minute of time,most accompanied by nausea,vomiting,sweating,etc.Vertigo drug treatment can reduce the detection rate of BPPV.BPPV patients with the reduction of treatment effect is good,and the earlier diagnosis and treatment,the more significant the effect.
