| Objective By comparing the clinicopathological characteristics and survival prognosis data of left colon cancer with right colon cancer patients, analyze the differences in epidemiological characteristics, clinical manifestations, pathological types and survival prognosis,to lay the foundation for developing accurate and individual treatment strategy of different locations of colon cancer and further research about colon cancer pathogenesis. Methods 277 colon cancer patients((128 cases of right colon cancer, left colon cancer 149)with complete clinical and pathological data from China-Japan Union Hospital of Jilin University from October 1,2014 to January 31,2017 were retrospectivelycollected in the study. The patient’s information including age, sex, clinical symptoms, pathological features, treatment was collected by taking the patient’s medical records and follow-up survey, and through home visits or telephone follow-up to master the prognosis of the patients survival. All patients were divided into the left half colon and right colon cancer, according to the different position of colon lesions by uniform analysis after data collecting. The data of the two groups were analyzed statistically, using c2 test for composition ratio difference with Kaplan-Meier Log-rank and Cox regression analysis for survival analysis and p <0.05 as a significantly difference. Results 1.There were 277 patients with colon cancer who were enrolled.Among them, the right-side colon cancer patients in 128 cases, accounting for 46.21%,the left-side colon cancer patients 149 cases, accounting for 53.78%.The proportion of left colon cancer in male patients was higher, while the proportion of right colon cancer in female patients was relatively high, the difference was statistically significant(p <0.05).2.The distribution of left and right colon cancer patients in different age groups was statistically significant,P <0.05;the onset age of the right-side colon cancer age was later.3.The patients with left-side colon cancer mainly manifested as bloody stool, stool and bowel habits change, and the patients with right-side colon cancer mainly manifested as abdominal pain, bloating, diarrhea, abdominal palpable and obvious mass. There were significant differences between the two groups <0.05,which is statistically significant.4.The mass size of the right-side colon cancer was larger than that of the left side, and the pathological type was mainly in the middle and low differentiation. The lymph node metastasis was more common,P <0.05.And there was no significant difference between the two in the distant metastasis,P> 0.05.5.The pathological stage of right colon cancer was relatively late, and the right and left sides of stage Ⅲ and Ⅳ accounted for 51.56% vs 29.56%.There was significant difference between the two groups(P <0.05).6.The prognosis of stage Ⅰ and Ⅱ was similar to that of left and right colon cancer,P> 0.05,and the prognosis of stage Ⅲ and Ⅳ right-side colon cancer was worse than that of left colon cancer(P <0.05).Conclusion 1.There were 277 patients with colon cancer who were enrolled.Among them, the right-side colon cancer patients in 128 cases, accounting for 46.21%,the left-side colon cancer patients 149 cases, accounting for 53.78%.The proportion of left-side colon cancer in male patients was higher, while the proportion of right-side colon cancer in female patients was relatively high.2.Age analysis shows that the incidence age of tumor in the right colon cancer was later than that of the left-side colon.3.The patients with left-side colon cancer mainly manifested as bloody stool, stool and bowel habits change, and the patients with right-side colon cancer mainly manifested as abdominal pain, bloating, diarrhea, abdominal palpable and obvious mass.4、The mass size of the right-side colon cancer was larger than that of the left side, and the pathological type was mainly in the middle and low differentiation. The lymph node metastasis was more common. And there was no significant difference between the two in the distant metastasis.5、TNM staging of right colon cancer was late, and TNM staging was earlier in the onset of left colon cancer.6、The prognosis of stage Ⅰ and Ⅱ was similar to that of left and right colon cancer, and the prognosis of stage Ⅲ and Ⅳ right-side colon cancer was worse than that of left colon cancer. |
