Objective : To evaluated the clinicopathological characteristics and survival prognosis with left-sided and right-sided colon cancer according to various cancer stages.Providing a reference for further treatment of colon cancer.Methods: A total of 178 patients with primary colon cancer undergoing surgery at the First Affiliated Hospital of Dalian Medical University from January 2009 to December 2011 were enrolled.We analyzed patient differences in terms of clinicopathologic features,disease free survival(DFS)in stage I-?,progression free survival(PFS)in stage IV and 3,5 year overall survival rate(OS%)of right-versus left-sided colon cancer.Results:1.Right-sided colon cancers were noted in 64(36.0 %)patients,including12,38,14 patients in stage I-?,? and IV,respectively.and left-sided colon cancers were noted in 114(64.0 %)patients,with 43,51,20 patients in stage I-?,? and IV.The average age between right-sided and left-sided colon cancer was 60.39±12.02 and61.23±10.52.Right-sided colon cancers exhibiting advanced CEA level(58.5%vs39.6%,P=0.028)and decreased UICC/AJCC stage I-?(18.8% vs37.7%,P=0.031).Analysis in various stages,right-sided colon cancer exhibiting advanced CEA level in stage I-?(66.7%vs32.6%,P=0.033).2.In all stages,Right-sided colon cancers were found to be significantly larger(68.7%vs31.6%,P < 0.001)and prone to lymphatic metastasis(78.1 % vs60.5%,P=0.017),poorly pathological type(23.4% vs 9.6%,P=0.016)in pathology.Analysis in various stages,right-sided colon cancers tend to be larger in stage I-?,?(83.3%vs27.9%,P=0.001 and 68.4%vs35.3%,P=0.002).In stage ?,right-sided colon cancer exhibiting poorly pathological type(28.9%vs9.8%,P=0.020).3.Patients with right-sided colon cancers had both poorer 3,5 year DFS rate and3,5 year OS rate than those with left-sided colon cancers in stage I-?.The 3,5 year DFS rate between right-and left-sided colon cancers was 70.0%,60.0% and78.3%,72.3%(P=0.043);The 3,5 year OS rate between right-and left-sided colon cancers was 82.0%,74.0% and 88.3%,80.9%(P=0.019);As for stage IV,mPFS between left-sided and right-sided colon cancer was 8.667 months and 7.100months(P=0.050),mOS between left-sided and right-sided colon cancer was 18.367 months and15.733months(P=0.034),multi-analysis shows tumor location was the predictor of stage IV colon cancer.4.Analysis by various stages revealed the 3,5 year DFS rate between right-sided and left-sided colon cancer was 65.8%,55.3% and 72.5%,64.7%(P=0.048).The 3,5 year OS rate between right-sided and left-sided colon cancer in stage ? was 80.0%,68.4%and 84.3%,74.5%(P=0.043);Regarding stage I-?,the 3,5 year DFS rate between right-sided and left-sided colon cancer was 83.3% ? 75.0%and 86.0% ? 81.4%(P=0.818).The 3,5 year OS rate between right-sided and left-sided colon cancer in stage I-? was 91.7%?91.7% and 93.0%?88.4%(P=0.979).The multi-analysis in stage I-? and stage ? have no significant difference.Conclusion:1.Right-sided colon cancer tend to be larger,poorly pathological types,exhibiting advanced CEA level?advanced AJCC level and prone to lymphatic metastasis.2.As for survival,left-sided colon cancer had better 3,5 year DFS rate and 3,5 year OS rate in stage I-?;Then,in stage IV,left-sided colon cancer had better mPFS and mOS than right-sided colon cancer.Tumor location can be a predictor of OS prognosis in stage IV.3.Analysis by various stages,right-sided colon cancer exhibiting advanced CEA level in stage I-?;In stage I-? and ?,right-sided colon cancer tend to be larger;poorly pathology type in stage ?.4.Analysis by various stages,left-sided colon cancer had better 3,5 year DFS rate and 3,5 year OS rate in stage ?;In stage I-?,there is no significant difference in 3,5year DFS rate and 3,5 year OS rate.
