A comparative study of anatomical locking plate and intramedullary fibular nailing for treatment of Danis-Weber B and C in the lateral malleolus fractureBackground The lateral therapy should be treated as intra-articular fracture,joint stiffness,in order to minimize the complications such as traumatic arthritis after surgery,treatment principle should be given fracture anatomical reduction,rigid fixation and early postoperative functional exercise.For a variety of treatment programs and the methods of internal fixation of lateral malleolus fracture,incision reduction plate fixation is associated with the common treatment,the operation requirements of medical technology is not high,in the direct manipulation of the fracture anatomical reduction,but the complications are relatively high.With the change of type MIPPO technology and deepening of the concept and material of internal fixation,intramedullary nail treatment of lateral malleolus peroneal provides alternative new fractures,because of its relatively small incision operation,low risk and postoperative complications,but the operation technology of medical demand is higher,can provide good imaging results and recovery function.Objective Danis-Weber B and C fractures of the lateral malleolus fracture,respectively using AO and Acumed comparative anatomical locking plate internal fixation of the fibular nailing related issues.Methods Analysis of 100 cases of ankle fractures in No.1 Hospital of Jilin University from 2014 to 04 in the year of 2016 with anatomical locking plate and intramedullary nailing in the treatment of ankle fractures in 01 cases.Remember the use of Danis-Weber type,the B and C type lateral malleolus fractures with anatomical locking plate in 60 cases,peroneal intramedullary nail in 40 cases,the cases of sex,age,soft tissue and joint dislocation,hospitalization,surgery time,bleeding,wound healing,infection,fracture,fracture healing,ankle joint function were observed.Results Two groups of surgeons were completed using AO lateral anatomical locking plate and intramedullary fibular Acumed needle in patients with gender,age,soft tissue and joint dislocation,there was no statistically significant difference(P>0.05).There was no significant difference in average length of stay and healing time(P>0.05).The standard of fracture reduction achieved anatomical reduction.Baird-Jackson ankle score in pain,walking ability,ankle stability,running ability,work ability,ankle joint activities,X-ray evaluation of fracture reduction results and total score difference was not statistically significant(P>0.05),there was statistical significance in operative time,blood loss,length of incision,incision complications(P<0.05),the a material and is influenced by the type of fracture using internal fixation combined with material,hospital fees temporarily no more meaningful.Conclusions Danis-Weber B and C in the lateral malleolus fractures Fractures,fibular nailing compared with anatomical locking plate is a more efficient way to less operative bleeding and reduce the complications of operative incision.For most of the distal fibula fracture,internal fixation is still fixed conventional means,wide indications,the anatomical reduction of the distal fibula fracture,and fixed reliable,good functional recovery,clinical operation is relatively simple,select steel plate as internal fixation materials,due to the fracture of surgical incision.The fracture and soft tissue conditions influence postoperative skin necrosis,infection and even plate exposure compared intramedullary pin increased.Intramedullary nailing for closed reduction,the operation of high technical requirements,need a certain hardware facilities and intraoperative fluoroscopy more,fracture reduction than anatomical plate slightly worse.Minimally invasive closed reduction to protect the soft tissue and less periosteal stripping,reduction of blood supply damage,conducive to fracture healing,reduce skin necrosis and infection;intramedullary fixation with longitudinal,biomechanical stability,early postoperative functional exercise,the two operation is easy to take out.The internal fixation was less discomfort,less demanding removal,but the final clinical healing time of two groups of fractures and the recovery of long-term ankle function were not significantly different.In clinical work,according to the economic conditions and compliance of patients,soft tissue conditions,fracture types and other specific circumstances to select the internal fixation methods and materials.
