Objective The aim of this study was to understand the prevalence of obesity and metabolic syndrome and analyze the epidemiological characteristics of metabolic syndrome in adult residents of Jilin Province,and then providing the basis for the implementation of the obesity intervention through identify the appropriate obesity indices and the optimal cut-off for metabolic syndrome.Methods A multidimensional stratified random cluster sampling method was used to draw face-to-face questionnaires,physical examination and laboratory tests of 16412 resident residents aged from 18 to 79 in 31 districts(cities)of Jilin Province in 2012.The survey was conducted using the "Personal Health Questionnaire" developed by the project team.The questionnaire mainly included basic demographic information such as sex,age etc.Physical examination included height,weight,waist circumference,hip circumference and blood pressure.Laboratory tests included blood lipids(High density lipoprotein,low density lipoprotein,cholesterol and triglycerides)and blood glucose testing.Quantitative data using the t test,qualitative data using the ?2 test.The LMS method was used to draw different indicators of fat changes with age in both sexes.Restricted cubic splines were used to test the dose response of different obesity indicators and metabolic syndrome.ROC analyses were used to find the optimal cut-off values of the various obesity indices and logistic analyses were used to evaluate the predictive ability.Results(1)The average of BMI was(24.34±3.62)kg/m2,WC was(84.67±10.44)cm,WHR was(0.88±0.07),WHt R was(0.50±0.06)in men,respectively;The average of BMI was(24.18±3.66)kg/m2 WC was(80.31±10.14)cm,WHR was(0.85±0.07),WHt R was(0.51±0.07),in women,respectively.There were statistically significant difference between men and women(P < 0.05),except for WHt R,men are higher than women.(2)Through the discovery of obesity and age analysis in different quantile levels.For men,weight gained the biggest change between 18~40 years old,41~60 years old tended to be stable,61~79 years old showed a decreasing trend;while women before the age of 65 were in the growth stage,after 65 years old weight tended to be stable and slightly decreasing trend.(3)The risk of Met S was increasing as the obesity index increases,but the young population was more tolerant than the middle-aged population and the elderly population,with an increase in the OR value relative to the slow,especially among the young women.On the whole,at any age,the female population was more tolerant than the male population,and its OR value increased relative to the slow.(4)The optimal cut-off values of BMI,WC,WHR and WHt R were 24 kg/m2,85cm(male)or 80cm(female),0.88(male)or 0.85(female),0.5,respectively.(5)Under different point of tangency,the OR values of men's BMI,WC,WHR,WHt R for metabolic syndrome were 9.01(95% CI: 8.00 ~ 10.15),16.15(95% CI: 14.21 ~ 18.35),8.97(95% CI: 7.93 ~ 10.15),11.49(95% CI: 10.16 ~ 16.13);AUROC values were 0.74(95% CI: 0.73 ~ 0.75),0.79(95% CI: 0.78 ~ 0.80),0.73(95% CI: 0.72 ~ 0.75),0.76(95% CI: 0.75 ~ 0.77),respectively;the OR values of women's BMI,WC,WHR,WHt R for metabolic syndrome were 6.28(95% CI: 5.63 ~ 7.00),11.41(95% CI: 10.09 ~ 12.90),5.37(95% CI: 4.81 ~ 6.00),10.68(95% CI: 9.33 ~ 12.23);AUROC values were 0.72(95% CI: 0.71 ~ 0.73),0.78(95% CI: 0.77 ~ 0.79),0.72(95% CI: 0.71 ~ 0.73),0.76(95% CI: 0.75 ~ 0.75),respectively.(6)WHt R had the best predictive and screening ability for hypertension,and WC had the best predictive and screening ability for dyslipidemia,diabetes mellitus and Met S in men;WHt R had the best predictive and screening ability for dyslipidemia,and WC had the best predictive and screening ability for hypertension,diabetes and the Met S in women.Overall,WC has better predictive and screening capabilities than other obesity indicators.Conclusion1.The prevalence of Met S,hypertension,diabetes and dyslipidemia in adult men residents is higher than that in women residents in Jilin Province.2.The change of weight shows a growth-stable-downward trend in men,while it is an increasing trend in youth and middle age,and in the elderly age which has a stable,downward trend in women.Overall,in the young age,the trend of weight increases quickly.In the middle age,the trend of weight gains slow down and the old age decreases slightly.It is suggested that weight control should start from the young age.3.Young people are more tolerant of obesity than older people,and adult women residents are more tolerant of obesity than men residents.4.WC has better predictive and screening capabilities than other obesity indicators for metabolic syndrome and its risk factors(hypertension,diabetes,dyslipidemia),followed by WHt R,the worst is WHR.5.Central obesity has more impact than other types of obesity on the metabolic syndrome and its risk factors(hypertension,diabetes,dyslipidemia),and mixed obesity has the greatest impact on the metabolic syndrome and its risk factors(hypertension,diabetes,dyslipidemia).
