| Objective The study analyzes the HIV and syphilis incidence rate among young men who have sex with men(YMSM)in Beijing,from 2011 to 2016 in a cohort study,and collect the information on the social demography,the knowledge and attitude about HIV related test and the sexual behavior.Our goal is to explore the influencing factors about HIV and syphilis seroconversion among YMSM and ultimately provide reference for policy makers.Methods There were 696 recruited eligible YMSM who participated in the study on yearly baseline between July 2011 to April 2016 in Beijing,and 567 YMSM completed follow-up surveys after six months.We collected information on the method of questionnaire and laboratory test.The application(survey)was developed by a software engineer to build database and the data were analyzed by SPSS 18.0statistical software package.The Logistic Regression Analysis was used to analyze the data.Results1.The baseline recruited 150 YMSM in 2011,149 YMSM in 2012,149 YMSM in 2013,138 YMSM in 2014,110 YMSM in 2015-2016.Statistically,many factors were of great significance including the age,marriage,the duration of not having Beijing resident card,student,education level,monthly income,gay man,sought male sex partners by Internet,the number of times of anal or mouth intercourse with male partners in the past month,the numbers of regular male partners in the last 3months,having anal intercourse with regular and casual male partners independently in the last 3 months,the consistent condom use with casual male partners in the last3 months,the condom use with casual male partners in the past month,the possibility of condom use with casual male partners in the next 3 months(P<0.05).2.115 YMSM were followed after 6 months and the tacking rate was 76.7% in2011,120 YMSM were followed after 6 months and the tacking rate was 80.5% in2012,125 YMSM were followed after 6 months and the tacking rate was 83.9% in2013,117 YMSM were followed after 6 months and the tacking rate was 84.8% in2014,and 90 YMSM were followed after 6 months and the tacking rate was 81.5%in 2015-2016.567 YMSM were followed after 6 months and the tacking rate was81.8% in five independent queues.The not having HIV related test in the last year(OR=0.458,95%CI=0.310-0.678,P<0.001)was worse to complete the tacking survey.3.5 YMSM had HIV seroconversion in 59.13 person-years tacking survey and the incidence rate was 8.46/100 person-years(95%CI=2.73-19.73)in 2011,2YMSM had HIV seroconversion in 61.08 person-years tacking survey and the incidence rate was 3.27/100 person-years(95%CI=0.37-11.82)in 2012,5 YMSM had HIV seroconversion in 70.28 person-years tacking survey and the incidence rate was 7.11/100 person-years(95%CI=2.29-16.60)in 2013,2 YMSM had HIV seroconversion in 69.82 person-years tacking survey and the incidence rate was2.86/100 person-years(95%CI=0.32-10.34)in 2014,4 YMSM had HIV seroconversion in 53.12 person-years tacking survey and the incidence rate was7.53/100 person-years(95%CI=2.03-19.28)in 2015-2016.The HIV seroconversion rate was 5.74/100 person-years(95%CI=3.40-9.08)in five independent queues and the difference was not significant(χ2=2.969,P=0.563).The self-consciousness of higher possibility of HIV infection(OR=3.835,95%CI=1.161-12.667,P=0.027)and unprotected anal intercourse in the last sex(OR=4.122,95%CI=1.579-10.757,P=0.004)were risk factors of HIV seroconversion.4.2 YMSM had syphilis seroconversion in 45.12 person-years tacking survey and the incidence rate was 4.43/100 person-years(95%CI=0.50-16.00)in 2011,6YMSM had syphilis seroconversion in 52.10 person-years tacking survey and the incidence rate was 11.52/100 person-years(95%CI=0.42-2.51)in 2012,6 YMSM had syphilis seroconversion in 58.91 person-years tacking survey and the incidence rate was 10.19/100 person-years(95%CI=0.37-2.22)in 2013,4 YMSM had syphilis seroconversion in 61.27 person-years tacking survey and the incidence rate was6.53/100 person-years(95%CI=1.75-16.71)in 2014,4 YMSM had syphilis seroconversion in 47.32 person-years tacking survey and the incidence rate was8.45/100 person-years(95%CI=2.27-21.64)in 2015-2016.The syphilisseroconversion rate was 8.31/100 person-years(95%CI=5.01-12.58)in five independent queues and the difference was not significant(χ2=1.867,P=0.760).The monthly income of 3000 yuan and below(OR=3.257,95%CI=1.269-8.355,P=0.014)and unprotected anal intercourse in the last sex(OR=6.668,95%CI=2.720-16.344,P<0.001)were risk factors of syphilis seroconversion.Conclusion1.The HIV and syphilis incidence rates were high among YMSM in Beijing,2011-2016.2.The tacking survey of not having HIV test in the last year was worse than having HIV test.3.The self-consciousness of HIV infection and unprotected anal intercourse in the last sex were independently associated with HIV seroconversion among YMSM.4.The monthly income and unprotected anal intercourse in the last sex were independently associated with syphilis seroconversion among YMSM. |
