Objective:Based on the monitoring data of pregnancy complicated with syphilis in Jiading District,Shanghai,the present study aimed to evaluate the pregnancy outcome of pregnant women with syphilis,to explore the factors affecting the outcome of pregnancy complicated with syphilis.Method:1.Retrospective analysis was performed to evaluate the the prevalence of syphilis during pregnancy in Jiading District,Shanghai,based on the the monitoring data from 2012 to 2015.2.A case-control analysis was conducted to compare the prevalence of adverse pregnancy outcomes and the influential factors.Case group was consist of 266pregnant women complicated with syphilis.According to the age and educational level of 1:1 match the same period 2012-2015 266 non-syphilis pregnant women,follow-up of pregnancy outcomes.3.Among 266 pregnant women with syphilis,group comparison were made to observe the impacts of gestational age,syphilis serological titers,the normative medication during pregnancy on the adverse pregnancy outcomes.4.The newborn infants born after syphilis were followed up to understand the occurrence of congenital syphilis.To explore the relationship between congenital syphilis and gestational age,serum titer of syphilis,whether standardized medication during pregnancy,and prophylactic medication after birth.5.SPSS statistical software was used to perform data analysis.Result:1.2012 to 2015,Jiading District,pregnancy prevalence of syphilis2012-2015 years of Jiading District a total of 96925 cases of maternal prenatal syphilis screening,screening out the pregnant women with syphilis in 266 cases,the detection rate of 0.27%pregnant women with syphilis.From 2012 to 2015 four years,syphilis detection rate increased from 0.66%to 2.30%of pregnant women,showing a rising trend(?~2=6.229,p=0.013).2.pregnancy adverse syphilis pregnancy outcomeCompared with non pregnant syphilis,syphilis in pregnancy'adverse pregnancy rate(11.95%vs.4.53%)increased significantly,the difference was statistically significant(?~2=9.349,p=0.002).Compared with non pregnant syphilis,pregnancy with syphilis incidence of preterm birth(7.52%vs.3.02%),the incidence of low birth weight(6.64%vs.2.26%)were significantly increased,the difference was statistically significant(p<0.05).3.pregnancy syphilis pregnancy outcome factorsThe incidence of adverse pregnancy outcomes in pregnant women with syphilis:The gestational age group<pregnant group<the third trimester group,the difference was statistically significant(?~2=8.047,p=0.018)In the untreated group,the difference was statistically significant(?~2=11.927,p=0.003);TRUST?1:8(17.78%)was greater than the TRUST<1:8 group(9.60%).The incidence of preterm labor of pregnant women with syphilis:TRUST?1:8 group(11.63%)was higher than that of group TRUST<1:8(8.06%);Not observed group(19.05%)>non-standard drug treatment group(7.87%)>specification group(0.00%);late pregnancy group(12.90%)>pregnant metaphase group(8.46%)>early pregnancy group(0.00%).Pregnancy with syphilis incidence of low birth weight:TRUST?1:8 group(11.63%)was higher than that of group TRUST<1:8(5.65%);Not observed group(14.29%)>non-standard drug treatment group(7.87%)>specification group(0.00%);late pregnancy group(9.68%)>pregnant metaphase group(7.69%)>early pregnancy group(0.00%).Conclusion:Pregnancy with syphilis can lead to increases of the incidence of preterm birth and low birth weight and the increases were closely associated with syphilis serology drop degree and medical intervention Standard medication during pregnancy can significantly control pregnanct syphilis-related preterm birth and low birth weight.The occurrence of adverse pregnancy outcome of pregnancy with syphilis was closely related to the first diagnosed gestational weeks and whether to standard medication during pregnancy.We should strengthen health education,Improve the patient's understanding of the disease,patients with early detection,early treatment,reduce the occurrence of adverse pregnancy outcomes.
