Background:Ovarian clear cell carcinoma is a distinctive entity of epithelial ovarian cancer,account for 5%-25% of ovarian epithelial carcinoma.The most significant clinical feature is resistant to traditional chemotherapy drugs,more susceptible of recurrence,extremity poor prognosis.At present,the treatment of clear cell carcinoma of the ovary contained of optimal cytoreductive surgery and the combination adjuvant chemotherapy of paclitaxel and platinum.At present,most of the clinical research data are small sample,single center,retrospective study,the data of survival and prognosis in different regions are discrepant,which are affected by the race,economic,and medical status.Objective:Clinical pathological data and prognosis of 50 patients with ovary clear cell carcinoma were retrospective analyzed.The Clinical pathological features and prognosis of patients with clear cell carcinoma of ovary were discussed.Method:From January 1,2011 to December 31,2014,total 60 patients with ovarian clear cell carcinoma admitted in the Second Hospital of Jilin University for surgical treatment,10 of them loss to follow-up.Retrospective analyze clinical pathological data and survival information of 50 cases of ovarian clear cell carcinoma with complete follow-up data,statistical software SPSS 19.0 was used for data processing and statistical analysis.Kaplan Meier curve and Log Rank test was used for univariate survival analysis.COX risk ratio regression analysis model was used for multivariate analysisto detect independent factor of prognosis,the results of statistically significant with P < 0.05.Result:1 From January 1,2011 to December 31,2014,a total of 60 patients admitted in the second hospital of Jilin University for operative treatmentand the postoperative pathologic diagnosis was ovarian clear cell carcinoma,accounting for 11.17% of ovarian cancerpatients.50 patients of them with complete follow-up data,with 88.23% of follow-up rate.2 Median age was 53 years old.Early disease accounted for 74%(stage Ⅰ accounted for 68%,stage Ⅱ accounted for 6%),and late stage accounted for 26%(stage Ⅲaccounted for 20%,stage Ⅳaccounted for 6%);20% cases with endometriosis;3 FIGO staging,satisfied with the reduction of tumor,malignant ascites,invasion of bilateral ovarian had significant difference on the survival time effect.4 Preoperative serum CA125 level,preoperative serum HE4 level,age,menstrual status,tumor size,preoperative ultrasound and postoperative pathological types chemotherapy and chemotherapy had no significant influence on prognosis.5 Multivariate analysis of satisfactory cytoreductive surgery,whether the invasion of bilateral ovaries and FIGO stage were independent prognostic factors.Conclusion:1 Clear cell carcinoma of the ovary was dominant in early stage,stageⅠ accounted for 68%,stage Ⅰ and stage Ⅱ accounted for 74%,and the incidence of endometriosis was 20%;2 FIGO staging,satisfied with the reduction of tumor,malignant ascites,invasion of bilateral ovarian had significant difference effect on the survival time.3 Preoperative serum CA125 level,preoperative serum HE4 level,age,menstrual status,tumor size,preoperative ultrasound and postoperative pathological types chemotherapy and chemotherapy had no significant influence on prognosis.4 Multivariate analysis of satisfactory cytoreductive surgery,whether the invasion of bilateral ovaries and FIGO stage were independent prognostic factors.
