Objective: To retrospectively analyze the medical records and follow-up data of 53 patients with clear cell carcinoma of ovary in our hospital,and to discuss the clinical features and prognostic factors of clear cell carcinoma of ovary.Methods: collected in January 2010-December 2015 in our hospital for the treatment of ovarian clear cell carcinoma patients with the clinical,pathological and follow-up data,and treated in the same period of low grade serous ovarian carcinoma,carcinoma of intrauterine membrane and mucous carcinoma patients and analysis of clinical data and survival of patients prognosis,count data by chi-square test,measurement data set using difference between two independent sample t test(normal distribution data)or nonparametric rank and inspection(non-normal distribution data),by Kaplan Meier survival curve analysis of the prognosis of patients with ovarian clear cell carcinoma.Results: There were statistically significant differences in preoperative CA125 elevation in patients with clear cell carcinoma of the ovary,those with endometriosis,and those with low grade serous and mucinous carcinoma(P < 0.05).The recurrence rate was statistically different from that of low grade serous carcinoma and intrauterine carcinoma(P < 0.05).The proportion of early stage patients and drug resistance rate of advanced stage patients were statistically different from that of low grade serous carcinoma(P < 0.05).Abdominal distension and abdominal mass as symptoms were statistically different from the other three types of epithelial ovarian cancer(P < 0.05).The 3-and 5-year survival rates were lower than those of intrauterine carcinoma(P < 0.05).Univariate analysis showed that the earlier the FIGO stage,the better the prognosis.Preoperative CA125 was normal(<35U/ml),negative lymph nodes,no greater omentum metastasis group,chemotherapy sensitive group,and the group without ascites were better than the positive group(P < 0.05).However,the difference of 5-year survival ratebetween menopause,tumor size and appendectomy was not statistically significant(P > 0.05).Multivariate analysis showed that lymph node metastasis,greater omentum metastasis,chemotherapy resistance and staging were all risk factors.Conclusion: patients with clear cell carcinoma of ovary were more in the early stage,and symptoms of abdominal distension,abdominal mass and other compression appeared earlier,accompanied by endometriosis.The 5-year recurrence rate was lower than that of serous carcinoma and intrauterine carcinoma,and the 3-year survival rate and 5-year survival rate were lower than that of ovarian intrauterine carcinoma.The rate of drug resistance is higher in advanced patients.Prognosis was associated with FIGO stage,preoperative serum CA125 level,lymph node and greater omentum metastasis,and chemotherapy resistance.
