Objective: To investigate the clinicopathologic features and prognosis of primary clear cell carcinoma of the liver(PCCCL), analyze the difference between PCCCL and common hepatocellular carcinoma(CHCC), and improve the diagnostic and treatment of PCCCL.Methods: The clinical and pathologic data of 30 cases of PCCCL and 60 cases of CHCC treated in Fujian provincial hospital from January 2009 to December 2013 was analyzed retrospectively.Results: There were 23 male cases and 7 female cases in the experimental group. The average age was 59 years. There was no statistical significance in gender, age, HBV infection, AFP, tumor size, tumor number, tumor capsule, tumor differentiation, cirrhosis and Child-Pugh score, while the vascular invasion rate was significantly lower in experimental group. The 1, 2 and 3 years overall survival(OS) rates for PCCCL was 93.8%, 79.5% and 70.7%, significantly better than those of CHCC(P=0.025). The 1, 2 and 3 years OS rates for PCCCL with and without vascular invasion were 66.7%, 33.3%, 33.3% and 100%, 90%, 78.8%, which were equal to the control group statistically(P=0.579, P=0.801). Cox proportional hazard model analysis showed vascular invasion was an independent prognostic factor for OS rate.Conclusions: In contrast to CHCC, PCCCL is less likely to have vascular invasion. The prognosis of PCCCL is better.
