Backgroud:With the prevalence of chronic obstructive pulmonary disease increased year by year,according to the World Health Organization reported that chronic obstructive pulmonary disease has become the cause of ischemic heart disease,stroke after the world's third largest cause of death.In China,according to Zhong Nanshan academician epidemiological survey data show that China's population over the age of 40 in the prevalence of chronic obstructive pulmonary disease as high as 14%Slowly blocked lung is not only a major factor affecting the quality of life of patients,but also increase the shortage of social medical resources,acute exacerbation of chronic obstructive pulmonary disease also exacerbated the situation,and the occurrence of acute exacerbations may also be to accelerate the progress of the disease One of the factors,but the current diagnosis of acute exacerbation of chronic obstructive pulmonary disease is still dependent on the patient cough,sputum,difficulty breathing and other symptoms,the original treatment can not effectively control the disease,need to change the treatment program These rely on patients or doctors Of the subjective judgments,there are some limitations,prone to misdiagnosis or misdiagnosis,so explore the diagnosis of acute exacerbation of acute lung disease is an objective indicator is necessary.Objective:To analyze the changes of blood biomarkers in AECOPD hospitalized patients and to explore the clinical significance of individual and combined detection of biomarkers.To compare the significance of biomarkers in the diagnosis of acute exacerbation of chronic obstructive pulmonary disease.To explore whether the indicators are relevant.Methods:Choose 50 AECOPD hospitalized patients from January 2016 to December 2016 in Liaoning Province People's Hospital,Fifty patients with stable period were selected as the control group.To measure the levels of serum markers,white blood cell count,neutrophil ratio,CRP,PCT,D-dimer and FIB,and evaluate the significance of the diagnosis AECOPD.Results:? The levels of WBC,N%,CRP,PCT,D-dimer and FIB in the AECOPD group were 9.84 ± 4.482,80.18 ± 10.880,59.40 ± 58.655,0.72 ± 0.818,752.57±454.692 and 4.29±1.143 respectively.The control group was 5.83±1.632,63.69±12.329,10.08±14.012,0.07±0.133,495.63±203.943,3.21±0.878,respectively.? The area under the ROC line of WBC,N%,CRP,PCT,D-dimer,FIB group were0.835,0.832,0.842,0.799,0.684,0.768,respectively.Adopted 8.64?71.35?8.50?0.163?726.22?3.55 as the cut-off point,the sensitivity,specificity,positive predictive value and negative predictive value of WBC were 60%,96%,94%and 71%respectively,the neutrophil ratio was 78%,74%,75%,77%,CRP was 86%?72%?75%?84%,PCT was 62%?98%?97%and 72%,D-dimer was 42%?90%?81%?61%,FIB was 74%?70%?71%?73%.The area under the ROC curve of the combined test N%and CRP was 0.864,and the area of N%and FIB was 0.855.They were higher than the single index.?In the AECOPD group,WBC was associated with N%,CPR,PCT,D-dimer,FIB(R = 0.530,0.434,0.496,0.377,0.395),N%and CRP,PCT,D-dimer,FIB(R = 0.680,0.214,0.564),PCT and D-dimer,FIB,and PCT,D-dimer and FIB were all correlated(R=0.680,0.214,0.564),PCT and D-dimer,FIB(R = 0.312,0.506),and the correlation coefficient between D-dimer and FIB is 0.543.In the control group,WBC was correlated with N%,CRP,PCT,D-dimer,FIB(R = 0.316,0.460,0.617,0.383,0.330),N%and CPR,PCT,D-dimer,FIB(R = 0.782,0.521,0.498),and between PCT and D-dimer(R ?0.782,0.521,0.498),and between PCT and D-dimer There was a correlation(R = 0.366,respectively),and the correlation coefficient between D-dimer and FIB was 0.438.Conclusion:The level of WBC,N%,CRP,D-dimer and FIB in AECOPD group were significantly higher than those in the SCOPD group,which could provide the basis for the diagnosis of disease.The area under the ROC curve of CRP was the highest than other groups.Combined test of N%and CRP,N%and FIB can effectively improve the AECOPD diagnostic rate.There was a linear correlation between some biomarkers,and the correlation between CRP and PCT was most pronounced.
