Analysis Of Therapeutic Effect Of Different Methods In Treatment Of Children With Fresh Monteggia Fracture

Objective To investigate the closed manipulative reduction and plaster external fixation for children of fresh Monteggia fracture of ulna,clinical effect analysis of open reduction and internal fixation of three treatment methods for fixed and single Kirschner wire fixation humeroradial joint with ulnar open reduction plate,provide a better diagnostic technique for the diagnosis and treatment of the disease,the theoretical basis and related basis,to improve the clinical children fresh Monteggia fracture healing effect.Method In this paper,through 48 cases collected from November 2014 to January 2017 in bone surgery the First Affiliated Hospital of Zhengzhou University,and all the data were retrospectively analyzed to evaluate the clinical effect.The 48 cases of fresh Monteggia fracture were randomly divided into A,B,C three groups,each group with 16 cases in each group were treated with A closed reduction and plaster external fixation,B group use of open reduction and plate fixation,group C with single Kirschner wire fixation of humeroradial joint with open reduction and internal fixation.Postoperative follow-up of 6 to 24 months(average 12 months),the three groups according to treatment of children with age and sex.Type,fracture healing time,results;according to the Mayo scoring system for three group fine rate;measurement in children with ipsilateral elbow flexion during follow-up(f/e),rotation(p/s)range,and measurement of lateral elbow flexion(f/e),The rotation(p/s)range,the resulting data were analyzed according to the statistical results;elbow joint function evaluation method(JOA)to calculate the elbow function score,and analyze the results statistically.The application of SPSS20.0 statistical software for statistical analysis of the results,a =0.05.level test.Results All of the patients were from 6 to 24 months after operation(average 12 months)follow-up,follow-up period in all patients without other injury and death.The three groups of data were retrospectively analyzed,three groups in age,gender typing,healing time was no significant difference in fracture(P>0.05).Three kinds of treatment methods of postoperative elbow function score good rate and the dislocation rate difference was statistically significant(P<0.05);the three groups were observed after treatment of lateral and contralateral flexion range and forearm rotation range,that there was statistical significance in A group and B group respectively extend scope and health on the side of the rotation range and yield difference;group C flexion and rotation range and the contralateral had no statistically significant difference;according to the scope of activities of functional scores were calculated,obtained three groups of elbow function score and contralateral were not statistically significant(P>0.05).Conclusions A,B,C three group treatment after treatment in healing time no difference,but the C group of elbow joint was higher than that of A,B two groups,and the dislocation rate using Kirschner wire fixation humeroradial joint(group C)can better prevent postoperative dislocation of radial head;A,B,C three groups the treatment methods in the treatment of elbow function score and contralateral but there is no difference between A group and B group after treatment compared with the contralateral elbow respectively rotation and flexion extension dysfunction after C treatment in children with elbow joint activities and contralateral no difference,postoperative flexion,rotation healed better;in summary,single Kirschner wire fixation of humeroradial joint with internal fixation for treatment in postoperative elbow flexion,pronation and supination of forearm and elbow function were excellent curative effect,high rate and can prevent postoperative dislocation of the radial head,in the clinical treatment of children can be used as fresh Monteggia fracture the Preferred method.
