Background Ischemic cerebrovascular disease(ICVD)has a high incidence,high recurrence rate,high risk and other characteristics[1],is a major risk factor for disability.Recently,more and more attention has been paid to the relationship between smoking and ICVD.Although the exact mechanism of ICVD and atherosclerosis has not been fully understood,but smoking has been recognized as one of the independent risk factors for cerebral infarction.Smoking index is equal to the number of cigarettes smoked per day multiply by number of years of smoking.Therefore,through the study of the relationship between smoking index and the degree and location of ICVD,we can quantify the relationship between smoking and cerebral vascular stenosis,and provide a theoretical basis for the prevention and treatment of ICVD.Objective To study the correlation between smoking index and the degree and location of ICVD cerebrovascular stenosis,to quantify the relationship between smoking and cerebrovascular stenosis,so as to provide a theoretical basis for more effective prevention and treatment of ICVD.Method ICVD patients hospitalized in Dalian Central Hospital between January 2011 and December 2016 were selected,collect samples(cases)related information,including age,gender,daily smoking number,years of smoking,hyperlipidemia,hypertension,diabetes mellitus,hyperhomocysteinemia(Hcy),alcohol,vascular stenosis degree and position etc.,207 male patients with smoking and hypertension and hyperlipidemia were included in this study,according to the head and neck CTA report,the stenosis degree and location of each patient were recorded,we compared the degree of stenosis between different groups in age,years of smoking,daily smoking number,smoking index and other factors have no significant difference,statistical software SPSS 21 for Windows was used for statistical processing,P<0.05 for the difference was statistically significant,and multiple stepwise linear regression was used to analyze the correlation between the factors and the degree of stenosis.Results1.Cerebrovascular stenosis groupThere were 76 cases in mild stenosis,25 cases in moderate stenosis,45 cases in severe stenosis and 61 cases in occlusion.There was no significant difference in the age,number of years of smoking,smoking number,smoking index,drinking and Hcy between the groups.2.According to the cerebral artery supply system,the patients were divided into two groups: anterior circulation group and posterior circulation group(1)Anterior circulation stenosis groupThere were 7 cases of anterior circulation without stenosis,111 cases in mild stenosis,26 cases in moderate stenosis,18 cases in severe stenosis and 45 cases in occlusion.There was no significant difference in the age,number of years of smoking,smoking number,smoking index,drinking and Hcy between the groups.(2)Posterior circulation vascular stenosis group1)There were 61 cases of posterior circulation without stenosis,60 cases in mild stenosis,22 cases in moderate stenosis,45 cases in severe stenosis and 19 cases in occlusion.There was significant difference between the groups in the number of years of smoking.In the age,smoking number,smoking index,drinking,Hcy showed no significant differences.2)Multiple stepwise linear regression analysis:Bounded by a=0.05 level,finally,the number of years of smoking can be entered into the regression model,and the degree of posterior circulation vascular stenosis was positively correlated with the number of years of smoking(B=0.256).3.According to the cerebral vascular stenosis,the patients were divided into two groups: intracranial group and extracranial group(1)Intracranial vascular stenosis group1)There were 17 cases of intracranial without stenosis,74 cases in mild stenosis,26 cases in moderate stenosis,34 cases in severe stenosis and 56 cases in occlusion.There was significant difference between the groups in smoking index,there was no significant difference between the groups in the age,the number of years of smoking,smoking number,drinking,Hcy.2)Multiple stepwise linear regression analysis:The smoking index were analyzed by multiple stepwise linear regression,bounded by a=0.05 level,finally,the smoking index can not be entered into the regression model,the results showed that there was no linear correlation between the degree of intracranial vascular stenosis and smoking index.(2)Extracranial vascular stenosis group1)There were 14 cases of extracranial without stenosis,121 cases in mild stenosis,20 cases in moderate stenosis,26 cases in severe stenosis and 26 cases in occlusion.There was a significant difference in the number of years of smoking and smoking index among the groups.In the age,the number of smoking,drinking,Hcy showed no significant differences.2)Multiple stepwise linear regression analysis:The above 2 factors were analyzed by multiple stepwise linear regression,bounded by a=0.05 level,finally,the smoking index could enter the regression model,and the degree of extracranial vascular stenosis was positively correlated with the smoking index(B=0.278).Conclusion1.There was no correlation between the degree of cerebrovascular stenosis and smoking index.2.There was no correlation between the degree of posterior circulation vascular stenosis and smoking index;there was a positive correlation between the degree of posterior circulation vascular stenosis and the number of years of smoking,the longer the number of years of smoking,the more serious the stenosis of the posterior circulation;there was no correlation between the degree of anterior circulation vascular stenosis and smoking index.3.The degree of extracranial vascular stenosis was positively correlated with smoking index,the greater the smoking index,the higher the degree of extracranial vascular stenosis;there was no correlation between the degree of intracranial vascular stenosis and smoking index.
