Objective: This study was aimed to examine tobacco use prevalence and quit smoking on the 15 years or above people of Nanchang and the influencing factors are studied in order to give the scientifically foundation for the further tobacco control.Methods: A multi-stage, geographically clustered sample design was used to produce city representative data in 14 participating cities. In the first stage, 25 communities were randomly selected as primary sampling units(PSUs), using selection probability proportional to size(PPS) sampling. In each community, if the number of household was over 1500, it would be divided into small areas containing 750 to 1500 households, and then randomly selected such a small PSU for the next sampling. In the second stage, 100 households were randomly chosen from the previously selected PSUs, using simple systematic sampling. In the third stage, one eligible household member from each selected household was then randomly chosen for an interview, using simple random sampling on the handheld computer designed for data collection. No proxy responding was allowed and no incentives were provided for participator. Descriptive and statistical analyses with percentages and 95% confidence interval(95% CI) were calculated using the complex survey analysis module of SPSS Version 19.0. Multivariate logistic regression modeling was also performed to identify influence factor for smoking and quitting smoking.Results:1. The prevalence of current smoking among Nanchang adults was 42.2% for males and 1.9% for females(OR 45.843, p<0.001). The highest prevalence was found in those aged 45-64 years(OR 1.628, p<0.001), and followed by those aged 24-44 years. The more knowledge of tobacco hazard, the less smoking rates. The prevalence of current smoking was higher in those whose family permit smoking indoor(OR 3.443, p<0.001). Most of the current smokers used manufactured cigarette, the user of using hand-rolled cigarette and other smoked tobacco were 0.3% and 0.1%, respectively. Only 0.1% of adults used smokeless tobacco products.2. There were 153 participants stopped smoking, males accounted for 94.4% after weighted, but the result of logistic regression analysis showed that females were more likely to quit smoking successfully than males(OR 1.433, p<0.001). The participants whose family had a ban of indoor smoking were more likely to quit smoking successfully(OR 2.729, p<0.001).3. In the last 12 months, 25.4% of current smokers attempted quit smoking at least one time. The smokers who smoked daily or whose family had a ban of indoor smoking were more likely to attempt quit smoking. The percentage of smokers who got advice for smoking cessation from doctors was only 48.6%. Only one in 10 smokers intended to quit smoking in the next 12 months. The percentage of smokers considering quitting smoking because of the cigarette pack warnings was only 22.5%. The smokers who had higher motivation were more likely to be those who had attempted quit smoking in the last 12 months(OR 17.276, p<0.001), had family ban of indoor smoking(OR 1.686, p<0.001), and had seen smoking control information in the last 30 days(OR 1.785, p<0.001).4. The non-smokers and ex-smokers had higher knowledge of tobacco hazard than current smokers. The percentage of current smokers who believe that cigarettes labeled as light, mild, or low tar are less harmful than regular cigarettes was 54.3%, which is great higher than non-smokers and ex-smokers with the percentage of 39.3% and 35.6% respectively.Conclusions:1. The prevalence of males smoking was still high in Nanchang city, especially the aged 45-64 years(56.1%).2. Most of smokers have no strong will to quitting smoking. The function of warning on the cigarette case play an unimportant role, and the doctor should give more advice for smoking people.3. The prevalence of current smoking was lower in those whose family with ban of indoor smoking, this ban helps to promote smokers trying to quit smoking.4. The understanding about specific health risks related to tobacco smoke among Nanchang adults, especially among current smokers, may still not be specific.
