| Cervical hyperextension injury is a kind of common mechanism of cervical spine injury.There are reports that such injuries accounted for 25%to 50%of all types of cervical injury,our statistics show that the rate as high as 59.7%,there are usually mild or occult bone damage,when no obvious osseous tissue damage or dislocation,often cause misdiagnosis.Therefore,it is of great clinical significance to analyze the factors related to the prognosis of patients with cervical hyperextension injury.At present,many scholars and research institutions of the cervical over-extension injury research and reports many,but the prognosis and related factors have not yet formed a unified opinionIn this study,the data we collected was based on those patients suffering from cervical hyperextension injury,who were treated in Yangzhou universer Hospital from 2013 to 2016,were carefully gathered and settled.On the basis of these data,we analyzed the possible relevant factors affecting the prognosis firstly.Then,we collected some X、CT and MRI images to analyze the possible relevant factors and the early prognosis of patients suffering from Cervical hyperextension injuryObjectives1.To simply summarize the characteristics of Cervical hyperextension injury and discuss the correlated factors influencing the early prognosis after Cervical hyperextension injury by a series of research.2.To investigate whether different signal changes of the spinal cord can predict the prognosis of patients suffering from Cervical hyperextension injury by analysis the operation of 17 patients treated in our hospital.MethodsThe study was based on a retrospective research on 78 cases of cervical hyperextension injury.The data we preliminary analyzed and generalized was from the patients who were received and cured in Yangzhou universal Hospital from 2013 to 2016.Firstly,we collected and preliminarily integrated the data and made the data quantization.Then,we used SPSS 17.0 to find the possible correlated factors influencing the early prognosis by the primary statistical methods.After that,the available factors(P<0.05)were picked up and analysised by Logistic regression to definite the real correlated factors.Results1.From January 2013 to 2016,we totally received 78 patients suffering from cervical hyperextension injury,among them,including 60 men and 18 female.The improvement rate of man group was 55%,the women group was 50%.The summit was mainly from 51 to 60 years,71 to 80 years;accounted for 42.3%of the total.Manual workers suffering from cervical hyperex--tension injury rates are relatively high compared to accident,accounted for 64.1%of the total.Traffic accident is the first reason causing the cervical hyperextension injury,and the following was fall and drop from high place.Among these factors,the severity of myelopathy,basic pathological changes before injury,cervical spine cord injury segment and operation is associated with the early prognosis of patients suffering from Cervical hyperextension injury.According to the American Spinal Injury Association(ASIA)classification standard,from A to D,the improvement rate respectively was 8.3%、28.6%、60.0%、83.3%.The patients with basic pathological changes are 51,and mainly from the older.The improvement rate of group with basic pathological changes before injury was 41.2%,the other group was 74.1%.The rate in the operated group was 94.1%,and the other group was 41.7%.2.After statistical analysis,we find that the different groups were variant in spinal cord damage degree,basic pathological change before injury and postoperative improvement rate.Before the injury base lesions and lower postoperative period.Surgical treatment and the recovery after surgery.Conclusions1.Gender,age,and cervical spine injury way stretch injury early prognosis of patients with no clear correlation.Spinal cord injury degree,the basic lesion before any injury,surgery or not stretch and cervical spine injury patients of early prognosis with clear relevance.Low of the spinal cord damage degree classification,low damage to the plane,no basic pathological changes and reasonable surgical intervention before injury protection factor is a good outcome.The pathogenesis of cervical vertebra have stretch injury,surgical timing,such problems as the choice of surgical approach should obtain further research,clear and refinement of surgical indications and timing,find it may be hidden problems.2.Surgical treatment is aimed at spinal cord decompression,maintain or recover fully cervical physiological curvature and the stability of the cervical spine.Early surgical decompression can relieve spinal cord edema,improve spinal cord blood circulation,avoid secondary lesions caused by spinal cord is chemia degeneration necrosis,surgical technique to improve the stability of the cervical spine can obtain immediate,help patients with early functional exercise and reduce various complications. |
