| Object :To explore the application of immediate prosthesis reconstruction(IPR),immediate latissimus dorsi reconstruction(ILDMFR)in the treatment of breast cancer with breast diarrhea mastectomy and cosmetic effect.Methods:Reviewin 32 breast cancer patients that had performed IPR or NSM plus ILDMR respectively in Department of Oncology;the First Affiliated Hospital of Bengbu Medical College between 2012/1 and 2016/1 and evaluate the applicable value and cosmetic results by telephone and outpatient.Results: Collected from January 2012 to January 2016,32 patients with nipple mastectomy plus immediate prosthesis reconstruction or immediate latissimus dorsi reconstruction of early breast cancer patients clinical data,including ILDMFR group of 18 patients,IPR group 14.The age of the ILDMFR group is(22~64)years,the mean age is(41.00 ± 9.65)years,the median age is 42 years;the age of the IPR group was(28~56 years),the average age(40.79 ± 8.52),the median age is 41 years old.The operation time in ILDMFR group is(180~326)minutes,with an average of(223.89±40.28)minutes,postoperative hospitalization time was(6-14)days,average hospitalization(8.61±2.33)days,hospitalization cost(13589~18924)yuan,the average cost(15938.89±1723.67);The operation time in IPR group is(170~340)minutes,with an average of(202.64±48.10)minutes,postoperative hospitalization time was(6~12)days,average hospitalization(7.57+1.02)(7.79±1.58)days,hospitalization cost(18254~24032)yuan,the average cost(20223.86±1741.01)yuan.Postoperative complications: 1 case of ischemic necrosis,2 cases of subcutaneous hemorrhage,1 case of reconstruction of breast atrophy in ILDMFR group while 1 case of capsule contracture,1 case of nipple ischemia and 1 case of local infection in IPR group.Postoperation aesthetic evaluation: the excellent rate is88.89%(16/18)in ILDMFR group and 85.71%(12/14)in the IPR group.Patients were surviving,1 case of bone metastasis in ILDMFR group,no local recurrence,no other postoperative complications.There was no significant difference between postoperative complication rate and cosmetic effect(P> 0.05).NSM plus IPR was less frequently in operation time and length of hospital stay(P <0.05).NSM plus ILDMR is inexpensive and has economic advantages,both of which are significantly different(P <0.05).Conclusion:For the choice of the appropriate patient to NSM plus IPR or ILDMR is safe and reliable,the two in the safety and beauty effects no significant difference.IPR of the advantages of breast reconstruction is simple,with a small surgical trauma,rapid recovery and many other advantages.ILDMFR of the low cost of breast,technology is simple,more in line with China’s national conditions,both methods deserve Extension. |
