Objective: By studying the changes of serum adipocytokines levels and related factors in patients with psoriasis vulgaris combined with metabolic syndrome(MS),explore the relationship between psoriasis vulgaris and MS and its possible pathogenesis.Methods: A total of 115 subjects were included in this study,including 30 cases of PSMS group,30 cases of PS group,30 cases of MS group and 25 cases of NC group.Serum Chemerin levels were measured by enzyme-linked immunosorbent assay(ELISA)in all subjects and comparing between groups,at the same time,the height,weight,waist circumference(WC),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(FP),fasting insulin(Fins),high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)of each group was tested,for all patients with psoriasis,additional investigation of psoriasis patients evaluated Psoriasis area and severity index(PASI)score,alcohol and tobacco history,medication history,family history.Partial indicators were correlated with serum adipokine Chemerin.All the results were analyzed using SPSS 16.0 for statistical analysis.Results:1.The levels of serum chemerin in PSMS group,PS group and MS group were higher than NC group,which in PSMS group was higher than PS group and MS group,the difference was statistically significant(P <0.05);2.The levels of serum hs-CRP,IL-6 in PSMS group,PS group and MS groupwere higher than those in NC group,PSMS group was higher than those in PS group and MS group,the difference was statistically significant(p<0.05);3.PSMS,PS,MS group BMI,WC,SBP,Fins,HOMA-IR were higher than NC group;PSMS group WC?Fins?HOMA-IR?LDL-C was higher than MS group;4.PSMS group of MS components distributed to abdominal obesity most,followed by high TG,hypertension,low HDL-C,high blood sugar;MS group of MS components distributed to hypertension most,followed by abdominal obesity,high TG,high blood glucose,low HDL-C;5.PSMS compared with PS group,the history of alcohol and tobacco,medication history and family history have no significant difference;PSMS group PASI score wassignificantly higher than PS group;6.The levels of serum Chemerin in PSMS group were significantly correlated with BMI(r=0.369),WC(r=0.428),TG(r=0.482),LDL-C(r=0.371),hs-CRP(r=0.519),IL-6(r=0.575),PASI(r=0.372)has a linear positive correlation(P<0.05);however SBP,DBP,HOMA-IR,TC,HDL-C had no significant correlation(P>0.05);7.Multivariate stepwise regression analysis showed that TG,WC,hs-CRP and IL-6 entered the regression equation with the concentration of serum Chemerin as the dependent variable and hs-CRP,IL-6,WC,TG as independent variables,which the greatest impact to the concentration of serum Chemerin is IL-6(? = 0.352),followed by hs-CRP(? = 0.302).Conclusion:1.Chemerin mainly affects the pathogenesis and progression of common psoriasis with MS by participating in inflammation,And its serum levels are closely related to obesity,lipid metabolism disorders,inflammation and severity of psoriasis;2.Psoriasis vulgaris and MS patients were in a certain degree of inflammation,with the highest degree of inflammation when both concurrent;3.Psoriasis vulgaris patients are in prevalence of metabolic disorders;Psoriasis vulgaris can increase the degree of abdominal obesity,insulin resistance and lipid metabolism in patients with MS,MS can also increase the severity of the disease in patients with psoriasis vulgaris.
