Objective:Using multivariate analysis of sputum syndrome factor and pathological degree in type 2 diabetes mellitus(T2DM)with obstructive sleep apnea syndrome(OSAS)patients,which to explore the role of sputum in the occurrence and development OSAS with T2DM and the influence factors causing changes.Research methods:Based on the method of cross section,collect 2015.07-2017.02 visits the First Affiliated Hospital of Xiamen University Department of Endocrinology and diabetes diagnosed type 2 diabetic patients,for patients with polysomnography,226 patients meet inclusion criteria with T2DM included with the OSAS.Used the "four diagnostic information collection form",which provided by the Fujian University of Traditional Chinese medicine(TCM)syndromes of TCM research base,to collection four diagnostic information and population data,western medicine clinical index in these 226 patients.To obtain the integral sputum syndrome factor by TCM syndrome differentiation system,according to the standard for evaluation of TCM syndrome factors divided into phlegm syndrome group and non-phlegm syndrome group,and analysis the correlation between the indexes and phlegm syndrome of TCM by comparison between groups and Logistic multiple regression.According to the criteria for judging the pathological grade of phlegm syndrome,the patients with phlegm syndrome were divided into 3 groups,compare the demographic data and the clinical indicators of western medicine in Phlegm syndrome pathological degree among groups,combined with linear regression analysis between phlegm syndrome and different indexes,to analyze the influence factors of increased pathological grade,and the data were analyzed by SPSS 20.0.Research results:1.In 226 cases of T2DM combined with OSAS,patients without sputum syndrome factor is 57 cases,then sputum syndrome factor cases were 169,accounting for all the subjects of 74.7%,among which the female subjects accounted for 36.6%,and male subjects is as high as 63.4%.2.Comparison of clinical data between the two groups,the phlegm syndrome group-s score higher than non phlegm syndrome group in BMI,waist circumference,HbA1c,C peptide,CHO,LDLC,AHI,snoring index and ESS score,there was significant difference(P<0.05);thenthere is no significant difference in blood rheology,liver and kidney function,UA,blood pressure between the two groups(P>0.05).3.The results of Logistic regression analysis showed that age,BMI,HbA1c,C peptide and ESS score were independent factors of phlegm syndrome,and the OR values were 1.052,1.274,1.460,0.356 and 1.226,respectively.4.Comparison of clinical data of sputum syndromefactor pathological degree,there are significant difference in age,HbA1c,CHO,LDLC,AHI,ESS score on between the spu-tum syndrome factor of 3 groups(P<0.05);gender,BMI,waist circumference,neck circumf-erence,family history of T2DM of course,blood rheology,liver and kidney function,FPG,C peptide,TG,HDLC,UA,SaO2,blood pressure,snoring index was no significant differen-ce between the four groups(P>0.05).5.Two variable linear sputum syndrome factor pathological degree regression analysis showed that age,BMI,HbAlc,CHO,LDLC,AHI,ESS and sputum syndrome factor integral positive correlation(P<0.01),C peptide and sputum syndrome factor score was negatively correlated.Multiple linear stepwise regression analysis showed that CHO,BMI,ESS score,age are the important factors of the degree of phlegm syndrome,linear regression equation Y=-78.91+14.03×CHO+3.52×ESS+2.14×BMI+1.54×age-1.59×course.Conclusion:1.Age,weight gain,poor glycemic control,daytime sleepiness is serious risk factor of produce phlegm in T2DM patients with OSAS,and good islet function is a protective factor to reduce the risk of produce phlegm.2.CHO,ESS scores,BMI,Ages,course of disease are influence factors of the increased phlegm pathological grade in T2DM patients with OSAS.HbA1c can only be used as a reference index for judging the degree of phlegm pathology.
