Objective: After a long time of development,total hip arthroplasty(THA)becomes one of the most successful procedure for end-stage arthritis in orthopedic surgery,which can improve function and quality of life in patients with hip disorders.However,it is usually associated with significant blood loss.Conventional compression dressing instruments,like Elastic bandage,have several deficiencies.In order to achieve the goal of providing a effective and convenient compression bandage therapy after total hip arthroplasty,we have invented the adjustable pneumatic compression device.In this experiment,we would to investigate the hemostasis effect of the adjustable pneumatic compression device after total hip arthroplasty.Methods: forty patients undergoing unilateral THA between August 2016 and April 2017 were randomly divided into observation group(adjustable pneumatic compression group,n=20)and control group(ordinary dressing group,n=20).There was no significant difference in age,height,weight,gender,ASA-class,pathogeny,disease duration,and preoperative hemoglobin between 2 groups.The total blood loss theoretical value,the postoperative drainage volume,the visible blood loss,the hidden blood loss,the number of patients receiving blood transfusion,and the related complications were compared between 2 groups.Results: No significant difference was found in operation time,intraoperativeblood loss and hospitalization time between 2 groups(t=0.337,P=0.738;t=1.014,P=0.317;t=0.306,P=0.761).The incisions healed by first intention in all patients,no postoperative complications of wound superficial infection,pressure ulcers and venous thrombosis occurred in 2 groups.One case had blisters in the observation group,no blisters occurred in the control group(?~2=1.026,P=1.000).Four incision subcutaneous hematoma in the control group,no incision subcutaneous hematoma occurred in the observation group(?~2=4.444,P=0.106).Circumference difference at mid-thigh level of two groups showed no significant difference(Z=0.992,P=0.321).After operation,no blood transfusion was given in observation group but 6 cases exist in control group,showing significant difference(?~2=7.059,P=0.020),and the total blood transfusion volume in control group was 1600 mL.There was no significant difference in preoperative blood volume,intraoperative blood loss and postoperative drainage volume between 2 groups(P>0.05),but the total blood loss theoretical value,the visible blood loss and the hidden blood loss in observation group were significantly less than those in control group(P<0.05).Conclusion: The adjustable pneumatic compression device could effectively reduce postoperative blood loss and the number of patients receiving blood transfusion.Although there was no significant difference,a trend towards less postoperative drainage volume was also observed in observation group.
